Violation descriptions and comments

In the meat department the seafood display cooler had co-mingling of scallops, clams, and mussels. The scallops, clams, and mussels must be separated from each other (i.e. plastic barrier)
There was a dial stem thermometer immersed in an ice bath in the deli area that was reading 40�°F
The 3 compartment sink in the produce area did not have a sanitizer dispenser that was dispensing at minimum 100ppm of Quats.
The integral AVB (Atmospheric Vacuum Breaker) on the hose bib faucet inside of the walk in dairy cooler, meat cutting room had hoses attached with spray nozzles. The hose bib in the walk in produce preparation cooler had a "Y" valve on it with shut offs attached to the faucet, thus allowing these AVB's to be left under constant pressure.
Need to segregate the open carton of ice cream that was found in the walk in grocery freezer due to no lid was found on the ice cream carton.
The caulking on three compartment sink in the meat department was found to be soiled. Rust was found on the shelving in the walk in deli cooler. Inside of the walk in deli cooler the shelving needs to have caps placed on the circular holes in the shelving to help prevent buildup of food debris in the holes.
(Bakery Department) There was paper debris was found in rolling salt bin. Several utensils were found to be soiled on the drying racks over the 3 compartment sink. The donut glazer was found to be in soiled condition.
(Bakery Department) The top of the metal cover for the bowl of the large "Lucks" mixer was found soiled. The price tag holders, and single use bag bins for the bread, and baked goods display case were soiled with bread crumbs. The walk in freezer, and walk in cooler door handles were found to be sticky, and soiled. (Meat Department) the condensation drain line was found to be soiled. (Deli Department) The holding bins for the single service food containers were found soiled. The shelving in the walk in deli cooler are starting to rust.
(Bakery Department) The hand sink caulking was found to be chipping away. The AVB on the faucet next to the three compartment sink was found to be broken. The condensation line in the walk in cooler was found dripping into a 5 gallon bucket. (Behind Dairy Cooler) The mop sink was found in soiled condition. (Meat Department) The caulking on the 3 compartment sink was found chipping away.
(Bakery Department) The back wall in the dry storage area was found to be soiled. The ceiling tiles by the vent system were found soiled. the floor by the bread slicing area was soiled as appears the bread slicer needs to be moved for thorough cleaning. The coving on the baked goods display and under the cashier stand are missing broken. (Grocery Walk In Freezer) The condenser line is dripping on the floor. The floor is soiled with packing, and food package debris. (Meat Department) The walls in the meat processing/ cutting room have holes in them. around the back wall and by the 3 compartment sink. The wall by the packaging equipment the wall was found to be soiled. (Produce Department) The ceiling tiles were found to be soiled. (Deli Department) Ice was found on the condenser line, and dripping on the floor of the walk in freezer.

Shellstock were co-mingled in ice in the seafood display case.
