Violation descriptions and comments

Raw ground beef and raw whole pork roasts were stored thawing next to lettuce in the walk in cooler.
The facility does not have a consumer advisory to inform the customers against the hazards of consuming raw or undercooked foods.
Employee beverages were stored on top of the fire extinguisher system and on top of the cooking oil in the back hallway storage area. Employee twist top bottles were stored in the walk in cooler on the same shelf as customer food.
An employee was observed not properly sanitizing the wire cheese slicer in the sanitizer solution of the 3 compartment sink for at least 60 seconds of contact time.
Threaded mop sink faucet does not have proper backflow prevention device installed on the faucet.
The sewer line of the men’s urinal was leaking on the floor.
There were flies present throughout the kitchen of the facility.
Ice buildup of non-potable water was found on the box of raw pork roasts from the condensation line of the condenser in the walk in freezer.
Cutting boards were found scored that were stored on the shelf in the food preparation area of the kitchen. The press plate for the tomato, and onion food slicer was found with a hole in the press plate, thus not an easily cleanable surface.
The ice machine door was found with a crack and missing corner of the lid. The opening lid did not have a seal attached to the lid. The caulking on the shelf above the cold top was found torn. The condensation line in the walk in freezer was found leaking water into the freezer. The sliding door beverage cooler in the back dry storage area had door holders that were made of metal pipe, which had duct tape on both ends of each pole not allowing for a smooth and easily cleanable surface.
The cutting boards were found stained in the storage are of the food preparation area. The table mounted can opener blade was found soiled. The cheese cutting wire was found soiled stored on the wall by the stand-up cheese reach in cooler.
The tub used for storing food bucket lids was found soiled on the bottom. The top of the shelf above the griddle top was found soiled. The plastic tubs used to store the food scoops were found soiled. The caulking on the left side of the 3 compartment sink was found torn. The top of the ice machine was found soiled. The caulking on the food preparation sink was found soiled.
Metal pans were found stacked wet throughout the storage areas of the kitchen, as well as the plastic food containers were found stacked wet in the back hall way storage area.
The scoop for the chips in the customer area did not have a handle attached.
The caulking on the hand sink near the front food serving area is torn or not present. The faucet of the hand sink in the front food serving area is not securely attached to the sink basin. The metal barrier on the hand sink in the front serving area is not sealed to the sink basin with caulking. The faucet in the preparation sink, as well as the faucet of the mop sink was found leaking water where the spout attaches to the faucet.
The dumpsters out back of the facility were found on a gravel surface, not stored on a surface that is nonporous; thus interfering the ability of cleaning the space where stored. The dumpsters did not have the drain plug installed thus allowing for pests to enter the dumpsters.
The bathroom wall where the urinal is attached has holes in the wall, large cracks were the FRP (Fiber Reinforce Board) is attached, and holes in the coving below the urinal. The ceiling tile was not properly secured in the hallway for the dry storage area. The ceiling tile in front of the walk in freezer was not secured in location. Raw wood was found in the door way to the back dry storage area of the kitchen. The floor tile was found chipped under the can storage rack in the hallway storage area. The stainless steel wall behind the 3 compartment sink had holes above the sink basins.
The light in the walk in freezer was found not working at the time of inspection.
In the back hallway on the equipment storage rack had employee bags stored on the rack next to the food equipment.
