Shielding to protect against broken glass falling into food shall be provided for all artificial lighting sources in preparation, service, and display facilities. MUST PROVIDE INSIDE WALK-IN COOLER.
All food establishments that display, prepare, or store potentially hazardous food shall have adequate refrigerated food storage facilities. WALK-IN COOLER AND ANY OTHER REFRIGERATION MUST BE ON/OPERATING AT PROPER TEMPERATURE OF 40F OR BELOW.
All food establishments that display, prepare, or store potentially hazardous foods shall have calibrated metal stem thermometers, provided and conspicuous, for refrigerated and hot food units. MUST PROVIDE FOR ALL UNITS.
Equipment and utensils should get proper exposure to the sanitizing solution during the rinse cycle. Bactericidal treatment shall consist of exposure of all dish and utensil surfaces to a rinse of clean water at a temperature of not less than 180F. MUST PROVIDE CHLORINE OR QUATERNARY SANITIZER WITH PROPER TEST STRIPS.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. ALL FLOORS MUST CLEAN. MUST REMOVE POOLING WATER FROM BASEMENT FLOOR. MISSING FLOOR TILES IN FRONT CUSTOMER AREA MUST REPLACE.
All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. ALL FRONT SHELVING MUST BE CLEANED AND PAINTED OR SEALED. ALL WALLS, CEILING AND FLOOR INSIDE WALK-IN COOLER MUST BE PROPERLY SEALED. NO OPEN SEAMS, PEELING TAPE, CREVICES OR HOLES AND GAPS.

All food shall be protected from contamination and the elements, and so shall all food equipment, containers, utensils, food contact surfaces and devices, and vehicles. OBSERVED CROSS-CONTAMINATION BETWEEN RAW FOODS AND COOKED OR READY--TO-EAT FOODS IN THE PREP COOLER AND THE WALK-IN COOLER. OBSERVED RAW BEEF AND RAW CHICKEN STORED ON THE WIRE RACK IN THE PREP COOLER ABOVE SLICED VEGETABLES. ALSO OBSERVED RAW SIDE OF BEEF STORED ON STORAGE SHELF OF THE WALK-IN COOLER ABOVE COOKED EGG ROLLS. OBSERVED EGG ROLLS STORED IN A CARDBOARD BOX TOP THAT WAS PREVIOUSLY USED TO STORE RAW CHICKEN. MANAGER REAARANGED THE PRODUCT IN THE PREP COOLER AND DISCARDED THE EGG ROLLS. CITATION ISSUED. CRITICAL VIOLATION 7-38-005A.
All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily. DETAIL CLEAN THE FOLLOWING ITEMS: MICROWAVE, VENTILATION HOOD AND FILTERS, FOOD STORAGE SHELVES IN THE CLOSET, STAINLESS STEEL SHELVES ABOVE THE 3 COMPARTMENT SINK, PREP TABLE, BASEMENT GREASE TRAP, WIRE STORAGE RACKS IN THE WALK-IN COOLER.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. REPLACE THE BROKEN FLOOR TILES IN THE WASHROOM. DETAIL CLEAN THE FLOOR OF THE STORAGE CLOSET NEXT TO THE DEEP FRYER, FLOOR BEHIND THE WOK AND DEEP FRYER, AND FLOOR OF THE WALK-IN COOLER.
The walls and ceilings shall be in good repair and easily cleaned. DETAIL CLEAN THE WALLS AND BASEBOARD IN THE FOOD PREP AREA. REGLUE THE FRP BOARD TO THE WALLS OF THE PREP AREA ABOVE THE 3 COMPARTMENT SINK AND NEXT TO THE WOK STATION. RESEAL THE WALL AROUND THE VENTILATION INTAKE IN THE RESTROOM.
Toilet rooms shall be completely enclosed and shall be vented to the outside air or mechanically ventilated. PROVIDE A SELF-CLOSING DOOR DEVICE FOR THE RESTROOM DOOR.
The area outside of the establishment used for the storage of garbage shall be clean at all times and shall not constitute a nuisance. OBSERVED EXCESSIVE AMOUNT OF GARBAGE AND WASTE OUTSIDE OF THE REAR EXIT AND THE STAIRCASE TO THE BASEMENT DOOR. MANAGEMENT INSTRUCTED TO MAINTAIN THE OUTSIDE PREMISES FREE OF EXCESSIVE GARBAGE SO AS NOT TO CREATE A PEST CONTROL ISSUE. CITATION ISSUED. SERIOUS VIOLATION 7-38-020.
All parts of the food establishment and all parts of the property used in connection with the operation of the establishment shall be kept neat and clean and should not produce any offensive odors. MUST FILL IN ANY RODENT BURROWS OUTSIDE OF THE RESTAURANT WITH CEMENT.
