Must provide drain stoppers to 2nd 3 compartment sink, no citation issued due to license inspection on 10/20/2011, business not open or operating for business. Walk in cooler door gasket in state of disrepair(broken), shall be replaced. Exposed raw wood in counter/bar areas, and storage areas shall be repair/sealed. must remove contact paper from storage shelving, cabinet walls. Non food contact surfaces of cooler shelving, cooking equipment, storage shelving not clean need cleaning(detailed). Floors that had damaged foor tiles, damaged carpeting, shall be replaced/repaired, floors in kitchen areas , front entrance area, front bar areas not clean need detailed cleaning(corners). Walls that had peeling paint, exposed plaster, contact paper, shall be repaired, sealed, and contact paper removed. No mop sink on premises, must provide mop sink to premises, per City of Chicago Health Code, also must provide exhaust fan to both bathrooms per City Of Chicago Health Codes. Leaking faucet of 3 compartment sink in kitchen prep area shall be reapir/replaced. Must provide thermometer to walk in cooler.
