All necessary control measures shall be used to effectively minimize or eliminate the presence of rodents, roaches, and other vermin and insects on the premises of all food establishments, in food-transporting vehicles, and in vending machines. OVER 50 MOUSE DROPPINGS WERE FOUND IN THE CAFETERIA UNDER THE RAMP TOWARDS THE ENGINEERS SHOP, BEHIND THE DOOR OF THE WEST CAFETERIA HEAT DECK, IN THE THIRD FLOOR TEACHERS LOUNGE PERIMETER AND AROUND THE 2ND FLR CHEMISTRY LAB WERE OLD FOOD ARTICLES WERE LEFT. CLEAN UP ALL EVIDENCE OF MICE AND REPAIR THE REAR GARBAGE DOOR EXIT, THE PEST STRIP AT HE BOTTOM OF THE DOOR WAS TORN OFF. FILL IN THE ACTIVE RODENT HOLES IN THE CAFETERIA BASEBOARDS.

All parts of the food establishment and all parts of the property used in connection with the operation of the establishment shall be kept neat and clean and should not produce any offensive odors. THE CHEMISTRY LAB IS FULL OF OLD ITEMS WHICH NEED TO BE REORGANIZED.

THE 3RD FLOOR GIRLS WEST WASHROOM HAS ONE SINK OUT OF ORDER, THE 2ND FLOOR GIRLS EAST FAUCET IS DAMAGED, THE LOWER LEVEL EAST GIRLS WASHROOM PLUMBING TO THE SINKS ARE SHUT OFF AND BEING REPAIRED AT THIS TIME. ONE URINAL IN THE LOWER LEVEL BOYS ROOM IS NOT WORKING. REPAIR ALL THE ABOVE PLUMBING. REPLACE THE MISSING STRAINER IN THE REAR CAFETERIA FLOOR DRAIN.
THE FLOOR DRAIN IN THE 3RD FLOOR GIRLS WEST WASHROOM IS MISSING TILES, REPAIR SAID FLOOR SO IT IS SMOOTH AND SANITARY.
THE ADA DOOR SWEEP ON THE SOUTH EAST CORNER OF THE BUILDING IS BECOMING WORN, REPLACE THE SWEEP.
THE WALK IN COOLER HAS DIRTY CEILINGS, RACKS AND FAN GUARD; WAH, RINSE AND SANITIZE THE ABOVE IN DETAIL.
THE WALLS ARE DAMAGED BETWEEN THE WALK IN COOLER AND FREEZER DOORS, THE PREP TABLE WITH THE GREEN PAINT ON IT IS PEELING, STRIP OFF THE PEELING PAINT.
