The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. Found floors not clean at storage areas.
All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. Found personal items(shoes,aprons etc.) stored on food shelving units.
All employees shall be required to use effective hair restraints to confine hair. Observed foodhandlers not wearing hair restraints.
A certified food service manager must be present in all establishments at which potentially hazardous food is prepared or served. Found no city of chicago sanitation manager on premises while handling potentially hazardous food.(handles deli sandwiches, chicken, ham, eggs etc.) serious violation. citation issued.

OBSERVED DIRT/FOOD BUILD UP INSIDE FLOOR DRAINS IN PREP AREAS AND INSIDE CABINET UNDER SODA MACHINE. MUST DETAIL CLEAN AND MAINTAIN INTERIOR OF ALL UNCLEAN FLOOR DRAINS ALSO MUST REGROUT ALL DAMAGED FLOOR TILES UNDER AND AROUND 3-COMPARTMENT SINK.
The walls and ceilings shall be in good repair and easily cleaned. OBSERVED MISSING CEILING TILES ABOVE ICE MACHINE. MUST PROVIDE AND INSTALL CEILING TILES WHERE NEEDED.
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. THE FOLLOWING IN NEED OF REPAIR- LOWER LEVEL HANDWASH BOWL (BY WALK IN COOLER) HOT WATER VALVE DAMAGED, 3-COMPARTMENT SINK FAUCET LEAKING AT BASE (LEFT SIDE FAUCET) AND MENS WASHROOM HANDWASH BOWL DRAINING SLOWLY. MUST REPAIR AND MAINTAIN SINKS.
Shielding to protect against broken glass falling into food shall be provided for all artificial lighting sources in preparation, service, and display facilities. WALK IN FREEZER LIGHT COVER MISSING. MUST PROVIDE AND INSTALL COVER FOR LIGHT BULBS.

INSTRUCTED MANAGER TO CLEAN AND MAINTAIN ALL UNCLEAN WALLS AND CEILING TILES.

PREP AREAS CUTTING BOARDS WITH PITS ON SURFACES AND HOT STATION PREP AREA REACH IN COOLER DOOR RUBBER GASKETS RIPPED. MUST REPLACE DAMAGED CUTTING BOARDS AND REPAIR DAMAGED DOOR GASKETS.

MUST DETAIL CLEAN AND MAINTAIN UNDERNEATH ALL PREP TABLES. MUST DETAIL CLEAN AND MAINTAIN THE KNIFE RACK IN LOWER LEVEL. MUST DETAIL CLEAN AND MAINTAIN THE FLOOR GREASE TRAP IN THE LOWER LEVEL PREP AREA.
MUST DETAIL CLEAN AND MAINTAIN THE FLOORS IN ALL CORNERS AND UNDER/AROUND ALL HEAVY EQUIPMENTS. CLEAN THE INTERIOR OF ALL FLOOR DRAINS THROUGHT OUT.
MUST DETAIL CLEAN THE VENTS ON THE 1ST FL ABOVE THE SERVICE AREA AND THE WALLS AROUND THE 3-COMP. SINK ON THE LOWER LEVEL.
MUST FIX THE LEAK AT THE BASE OF THE 3-COMP. SINK AND AT THE BASE OF THE FAUCET AND PIPE ON THE EXPOSED HAND WASHING SINK IN THE REAR (BY DRY STORAGE) IN THE LOWER LEVEL PREP AREA.
