All food establishments shall be provided with an adequate supply of hot and cold water under pressure properly connected to the city water supply. NO HOT WATER IN WASHROOMS, INSTRUCTED TO PROVIDE.
All necessary control measures shall be used to effectively minimize or eliminate the presence of rodents, roaches, and other vermin and insects on the premises of all food establishments, in food-transporting vehicles, and in vending machines. NO LOG BOOK, INSTRUCTED TO PROVIDE A LOG BOOK WITH ALL REQUIRED INFO.
The area outside of the establishment used for the storage of garbage shall be clean at all times and shall not constitute a nuisance. OUTSIDE GARBAGE OVERFLOWING AND BROKEN LID, INSTRUCTED TO CORRECT.
All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. MISSING OUTLET COVERS ON NORTH WALL PREP AND WOMEN'S WASHROOM, INSTRUCTED TO CORRECT. PROVIDE STORAGE SHELVES FOR WIC AND WIF.
The walls and ceilings shall be in good repair and easily cleaned. REPLACE/REPAIR CEILING TILES IN STORAGE ROOM.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. MISSING FLOOR TILES IN STORAGE ROOM, INSTRUCTED TO REPLACE.
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. INSTALL THE ICE MACHINE WITH BACK FLOW DEVICE.

(7-38-030) INADEQUATE PLUMBING. NO HOT WATER AT HANDSINKS, IN BOTH EMPLOYEE/CUSTOMER BATHROOMS AT REAR OF BUILDING-MUST PROVIDE. When toilet and lavatory facilities are provided for the patrons of food establishments, such facilities shall be adequate in number, convenient, accessible, properly designed, and installed according to the municipal code.
FOUND A WATER LEAK ON PIPE UNDER THE 3-COPM. SINK (MIDDLE COMPARTMENT)-MUST FIX. Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health.
INTERIOR FLOOR OF FRONT COOLERS MUST BE FREE OF SPILLS/FOOD DEBRIS. INTERIOR PANEL OF ICE MACHINE NEED CLEANING ALSO. All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily.
