Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. Food shop prep area 1-compartment sink pipe leaking. Must repair and maintain.
All food establishments that display, prepare, or store potentially hazardous food shall have adequate refrigerated food storage facilities. Found 2nd floor prep area 4-door cooler not maintained, ambient air temperature at 45.5 F. Instructed manager to repair and maintain cooler. Citation issued. Ticket #H000058027 15.
All cold food shall be stored at a temperature of 40F or less. Found potentially hazardous foods held at improper tempertures between 45.5 F and 50 F- Tuna, Egg Salad, Ham, Cheese, etc. Instructed manager all potentially hazardous foods must be held at 40 F or below if held cold. Manager voluntarily discarded all out of temperature foods at this time. Citation issued.
All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. Found prep reach in coolers door rubber gaskets in poor repair, ripped. Must replace/repair all damaged door gaskets.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. Must detail clean floor under and around all shelves, along corners and walls throughout Bakers storage room.
All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily. Observed slight lime build up inside both ice machines. Must clean and maintain equipment.

All necessary control measures shall be used to effectively minimize or eliminate the presence of rodents, roaches, and other vermin and insects on the premises of all food establishments, in food-transporting vehicles, and in vending machines. OBSERVED LIVE FRUIT FLIES ON THE WALLS AND CEILINGS AT THE JUICE BAR, 1ST FLOOR DISHWASH AREA AND BASEMENT HALLWAY STORAGE AREA. APPROXIMATELY 50 LIVE FRUIT FLIES WERE SEEN THROUGHOUT THESE AREAS. ADDITIONAL PEST CONTROL SERVICE IS NEEDED TO ELIMINATE OR MINIMIZE THE PEST ACTIVITY. SERIOUS VIOLATION 7-38-020.

MUST SEPARATE COLD PLATE AND DRAIN LINES FROM THE CONSUMABLE ICE.

ORKIN PEST CONTROL WAS HERE ON 6/22/11 AND 6/27/11. KITCHEN EMPLOYEES ARE CURRENTLY USING VACUUMS TO REMOVE THE FLIES FROM THE AREAS LISTED. SUPERVISOR NOTIFIED. CRITICAL VIOLATION 7-42-090.
