All necessary control measures shall be used to effectively minimize or eliminate the presence of rodents, roaches, and other vermin and insects on the premises of all food establishments, in food-transporting vehicles, and in vending machines. Over 100 mouse droppings were found around and under shelving and baseboards throughout the 1st and 2nd floor classrooms and closets. Remove all evidence of pest activity, shampoo the soiled carpets and seal up all areas of possible harborage including the holes in the baseboards in the front entry waiting area and electrical closets. Seal the outer openings in the mechanical room on the first floor and in the adult program room.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. The classroom, store and office floors are full of mouse droppings. Clean and maintain the floors and baseboards so they are smooth and sanitary from corner to corner Shampoo carpets to remove rodent urine and fecal buildup.
All parts of the food establishment and all parts of the property used in connection with the operation of the establishment shall be kept neat and clean and should not produce any offensive odors. Remove or properly store the supplies and personal items that are stored on the floors so regular daily cleaning can be done and easily inspected from corner to corner.
All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. All adjustable shelving needs to be raised at least 6 inches off of the floor so cleaning and inspection can be done easily.

No evidence of mouse activity was found at this time, Orkin Pest Control was on premises on 1-25-11 and will trap each room over the weekend. Holes filled in and outer openings were sealed.
All shelving has been raised for easy cleaning.
The flors and carpets are now clean.
Clutter and personal items have been removed.

The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. THE FLOORS NOT CLEAN IN THE STORE ROOMS 1ST AND 2ND FLOOR AND IN THE WATER HEATER ROOM ALONG THE WALLS AND IN CORNERS. INSTRUCTED TO CLEAN SAME.
The flow of air discharged from kitchen fans shall always be through a duct to a point above the roofline. CLEAN THE VENTS IN THE KITCHEN HAS DUST BUILDUP.
Cleaning equipment shall be properly stored away from food, utensils, equipment, and clean linens. LARGE AMOUNT OF CLUTTER IN THE SECOND FLOOR STORE ROOM HAS WOOD, BOXES OLD UNUSED EQUIPMENT ECT. INSTRUCTED TO CLEAN AND ORGNIZE SO NO INSECT OR RODENT HARBORAGE.
