All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily. INSTRUCTED TO CLEAN DEBRIS AND STAINS ON THE FOLLOWINGS:2COMP.SINK,MOP SINK,AND DISH FLUSHER SINK ON DISH MACHINE.
All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. MUST REMOVE WALK IN COOLER DRAIN HOSE IN 3COMP.SINK AND ROUTE ELSEWHERE ,AND PROVIDE SPLASH GUARD PROTECTION ON ICE MACHINE BY DISH MACHINE FLUSHING SINK. ALSO REPAIR RUST ON WALK IN COOLER FLOORS AND ON ICE MACHINE.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. INSTRUCTED TO CLEAN DEBRIS ON FLOORS IN THE OFFICE.
Between uses and during storage ice dispensing utensils and ice receptacles shall be stored in a way that protects them from contamination. INSTRUCTED TO PROVIDE AN CONTAINER WITH LID COVER FOR ICE SCOOP AT ICE MACHINE.
Shielding to protect against broken glass falling into food shall be provided for all artificial lighting sources in preparation, service, and display facilities. MUST REPLACE BROKEN LIGHT SHIELD IN KITCHEN.
Cleaning equipment shall be properly stored away from food, utensils, equipment, and clean linens. All parts of the food establishment and all parts of the property used in connection with the operation of the establishment shall be kept neat and clean and should not produce any offensive odors. INSTRUCTED TO REMOVE CLUTTER ON THE TOP OF ICE MACHINE,MAINTAIN CLEAN AND FREE OF LITTER. ALSO MUST ORGANIZE CLUTTER IN THE OFFICE.

Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. REPAIR ON EXPOSE HAND WASH SINK IN KITCHEN.
