All necessary control measures shall be used to effectively minimize or eliminate the presence of rodents, roaches, and other vermin and insects on the premises of all food establishments, in food-transporting vehicles, and in vending machines. found observed rodent droppings(60 )in rear storage areas under shelving, and by rear exit door. screen door not rodent proofed, door had 1/2 inch gap on bottom of door.citation issued#H000063580 with a court date of 12/23/2010, at 400 w superior in room #112 at 9:00.
cooler shelving that had rust, peeling paint, shall be repair/replaced.

previous serious violation from 11/10/2010 not corrected, still found evidence of rodent activity (over 100 rodent droppings throughout premises)in prep area, storage areas, display areas,under, and on shelving where food is stored at this time of canvass re-inspection on 11/17/2010. Critical violation. Citation #H000063581 issued. Must remove all evidence of rodents, wash and sanitize all surfaces and have premises serviced by pest control company. Comply with all orders regarding sanitation and maintanence of premises.

All cooler shelving that had rust, peeling paint, shall be repair/replaced.
Food and non-food contact surfaces of cooler shelving, door tracks, storage shelving, ice machine, meat slicer,meat grinder need detailed cleaning. Display and storage shelving/racks throughout premises need detailed cleaning, and sanitizing. Remove all dust, debris, spillage etc.
Floors under heavy equipment, storage shelving, storage rooms, offices not clean, need detailed cleaning(corners) through-out. Floors in walk in cooler, walk in freezer need detailed cleaning. Floors throughout display areas under shelving need detailed cleaning and rodent droopings removed, also detail clean floor under display coolers throughout store/premises. Repair areas of floor where tiles are missing or broken.
Damaged ceiling tiles, stained, missing ceiling tiles throughout premises, shall be repair/replaced. Damaged ceiling in rear prep area shall be repair. Walls that had gaps, holes peeling paint throughout premises shall be repair. Walls and ceiling through-out premises must be cleaned, repaired and maintained. Install 16 mesh to the inch screens at rear screen door (tight-fitting) and and wall vents.

exhaust vents(ventilation) and fans in walk in cooler, coolers, kitchen areas, sales floors must be cleaned (remove dust build-up) and re-finish if rusty. Must restore hot and cold running water to all sinks. Clean and maintain all sinks. No ventilation hood above gas stove in rear. Must install hood or remove stove.
Elevate all product/stock 6 inches off of floor to prevent pest harborage. Excess debris and clutter and equipment shall be removed from rear storage areas, stock items must be stored six inches off floor.

DAMAGED CEILING AND WALLS IN THE REAR AREA BY THE EXIT DOOR, INSIDE THE ROOM WHERE THE ICE MACHINE IS LOCATED AND INSIDE THE WASHROOM.MUST REPAIR AND MAINTAIN. PEELING PAINT ON THE CEILING INSIDE THE WASHROOM. MUST REMOVE PEELING PAINT. STAINED CEILING TILES THROUGHOUT THE STORE ,MUST REPLACE AND MAINTAIN.
LEAKING ON THE FAUCET CONNECTOR OF THE 3 COMPT SINK. MUST REPAIR AND MAINTAIN.
STANDING WATER INSIDE THE ICE MACHINE ROOM. MUST DRY THE FLOOR. MUST CLEAN THE FLOOR IN THE BACK AREA UNDER THE COMPRESSOR. INSIDE THE WALK IN COOLER AND WALK IN FREEZER. ELEVATE ALL BOXES OF MEAT OFF THE FLOOR OR PROVIDE SHELVING FOR THE BOXES OF MEAT INSIDE THE WALK IN FREEZER.
MUST PROVIDE THERMOMETER AND MUST BE VISIBLE INSIDE THE WALK IN COOLER AND WALK IN FREEZER AND DISPLAY COOLER AND REACH IN COOLER.
