All cold food shall be stored at a temperature of 40F or less. Found approx 10-lbs of raw ground beef (Value=$17.00), on top shelf of pizza prep cooler at 53.4f. Instructed to preperly dispose off & maintain all cold food at 40f or below. Citation issued.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. Most sink drains at dishroom, floors along wallbase under bar sinks need cleaning.
The walls and ceilings shall be in good repair and easily cleaned. Fan guards/compressors of most coolers must be free of dust build-up. Water-stained ceiling tiles in ladies bathroom must be replaced.
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. Both of the customer bathromm vents are not properly working, musr fix. Leaking hot water line under rear prep sink, leaky faucet at handsink by cook's line must both be fixed.

All rooms in which food or drink is prepared, or in which utensils are washed, shall be lighted so that a minimum of 50-foot candles of light is available on all work surfaces. OBSERVED BURNED OUT CEILING LIGHT BULBS IN PREP AREA. MUST REPLACE WITH WORKING BULBS.
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. THEN PUT INTO REACH IN COOLER UNTIL A ORDER IS MADE FOR SANDWICH THEN REHEATED IN SAME CONVEYOR OVEN. MANAGER VOLUNTARILY DISCARDED OUT OF TEMPERATURE POTENTIALLY HAZARDOUS FOODS AT THIS TIME. MISSING BASEBOARD TILES TO THE RIGHT OF BEER WALK-IN COOLER DOOR & ON A CORNER WALL ACROSS THIS SAME DOOR. DUSTY BATHROOM VENTS MUST BE CLEANED & FOOD STAINS ON WALL & CEILING AROUND WAIT STAFF STATIONS BY THE KITCHEN DOORS MUST BE CLEANED ALSO.
HOT WATER KNOB AT A DUMP SINK IN THE BAR IS NOT WORKING; THE FAUCETS AT 1-COMP SINK AT REAR PREP TABLE IS LEAKING-MUST FIX.
