OBSERVED EVIDENCE OF RAT DROPPINGS IN REAR REPACKING AND BUTCHER'S AREA,AND UNUSED WASHROOM: 15 RAT DROPPINGS WAS OBSERVED UNDER THE HOT WATER TANK,5 RAT DROPPINGS WAS OBSERVED INSIDE THE WALK-IN COOLER (UNDER THE PLASTIC CRATES)WHICH IS USED ONLY FOR DRY FOOD.20 OR MORE DROPPINGS INSDE THE WASHROOM / STORAGE FOR FOAM TRAYS AND OTHER ITEMS. 5 DROPPINGS UNDER THE BUTCHER'S COOLER,IN REAR AREA.STRONG FOUL ODOR THROUGHOUT THE PREMISES.THREE HOUSE FLIES WERE OBSERVED IN REAR REPACHING/BUTCHER'S AREA.OUTER OPENINGS NOT PROTECTED:ROLLED UP SIDE DOOR HAS A ONE INCH GAP AT BOTTOM DOOR(NORTH-EAST SIDE). Containers and utensils shall be inverted, covered, or otherwise protected from contamination until used. All single-service drinking straws and containers shall be discarded immediately after use. INSTRUCTED NOT REUSE FOAM TRAYS(DO NOT WASH THEM,ONCE USED MUST BE DISCARDED).

Shielding to protect against broken glass falling into food shall be provided for all artificial lighting sources in preparation, service, and display facilities. NEED TO CLEAN INSIDE THE LIGHT SHIELD IN SELLING AREA(DEAD INSECTS)
