OBSERVED 1 OF 2 WASHROOM HANDSINKS WITHOUT HOT WATER IN THE 2ND FLOOR BOY'S RESTROOM. MUST REPAIR AND PUT SINK OUT OF ORDER UNTIL REPAIRED. MUST CAULK AROUND THE HANDSINK IN THE 2ND FLOOR GIRL'S RESTROOM.

The walls and ceilings shall be in good repair and easily cleaned. REPAIR PEELING PAINT FROM ABOVE THE EXPOSED HAND SINK IN KITCHEN.REMOVE SPIT BALLS ON CEILING AT 2ND FLOOR BOY'S WASHROOM.
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. MUST PROVIDE A BACK FLOW PREVENTER IN ALL MOP SINK IF GARDEN HOSE IS ATTACHED AND REMAINS ATTACHED. REPAIR MECHANICAL DRAIN HANDLE UNDER THE 3 COMPARTMENT SINK(SANITIZE COMPARTMENT),WHEN WATER IS RELEASED.
