All dishwashing machines must be of a type that complies with all requirements of the plumbing section of the Municipal Code of Chicago and Rules and Regulation of the Board of Health. Found the 3-comp sink at the bar is clogged. Instructed to fix, sanitize & maintain & use the dishmachine or 3-comp sink in dishroom.
All necessary control measures shall be used to effectively minimize or eliminate the presence of rodents, roaches, and other vermin and insects on the premises of all food establishments, in food-transporting vehicles, and in vending machines. Found over 50-mice droppings along wallbase to the left of the ice machine. Must seal any openings on walls/ceiling in this room, clean/sanitize the area & contact P.C.D.
A separate and distinct offense shall be deemed to have been committed for each Minor violation that is not corrected upon re-inspection by the health authority. Following minor violations from 3/5/09 visit, not corrected this time. (#33-exterior of cooking equipment, interior floors & gaskets of coolers in bar & kitchen; #34-floors behind/under kitchen equipment & in basement must be kept clean & all items/stock must be elevated off the floor in basement storage areas; #35-missing ceiling tiles above dishmachine, interior attachments to hood above cooking equipment, condensers/compressors/fan guards of most coolers must be free of heavy dust/grease build-up).
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. Floors thruout bar area must be kept clean & free of food debris/dust build-up.
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. Faucet sprayer at 3-comp sink must be fixed & stay above the water level of the sink to prevent back siphonage.

All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily. THE FOLLOWING NOT CLEAN- OBSERVED ACCUMULATED FOOD DEBRIS INSIDE PREP AREA REACH IN COOLERS RUBBER DOOR GASKETS, BAR GUN HOLDERS, EXTERIOR OF SINKS AND COOLERS IN PREP AND DISHWASH AREA AND INTERIOR OF ICE MACHINE WITH SLIGHT LIME BUILD UP. MUST CLEAN AND MAINTAIN ALL UNCLEAN EQUPMENT.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. OBSERVED BEER/BOTTLE CAPS ON FLOOR AT BAR AND FOOD DEBRIS UNDER AND AROUND EQUIPMENT AT PREP AREA/STORAGE AREA AND BASEMENT. MUST DETAIL CLEAN FLOOR THROUGHOUT AND MAINTAIN.
The walls and ceilings shall be in good repair and easily cleaned. OBSERVED FOOD DEBRIS ON WALL BEHIND/AROUND DISHMACHINE. MUST CLEAN ALL UNCLEAN WALLS AND MAINTAIN.
All rooms in which food or drink is prepared, or in which utensils are washed, shall be lighted so that a minimum of 50-foot candles of light is available on all work surfaces. MUST REPLACE BURNED OUT CEILING LIGHT BULBS AT PREP AREA.
All food establishments that display, prepare, or store potentially hazardous foods shall have calibrated metal stem thermometers, provided and conspicuous, for refrigerated and hot food units. MUST PROVIDE INTERIOR THERMOMETER FOR FOOD WALK IN COOLER.
All parts of the food establishment and all parts of the property used in connection with the operation of the establishment shall be kept neat and clean and should not produce any offensive odors. 1ST FLOOR STORAGE ROOM AND BASEMENT CLUTTERED. MUST ELEVATE ALL ITEMS 6' OFF FLOOR AND AWAY FROM WALLS OR REMOVE TO PREVENT POTENTIAL PEST HARBORAGE.
All employees shall be required to use effective hair restraints to confine hair. FOOD HANDLER NOT WEARING HAIR RESTRAINT. MUST PROVIDE HAIR RESTRAINT FOR FOOD HANDLER(S).
