All employees shall be required to use effective hair restraints to confine hair.
All food not stored in the original container shall be stored in properly labeled containers. PREP FOODS INSIDE WALK IN COOLER MUST BE LABELED.
Containers and utensils shall be inverted, covered, or otherwise protected from contamination until used. All single-service drinking straws and containers shall be discarded immediately after use.
All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. OBSERVED 2 BROKEN DOORS FOR MEAT DISPLAY COOLER, MUST REPLCE DOORS AND MAINTAIN.
All utensils shall be thoroughly cleaned and sanitized after each usage. THE FOLLOWING NOT CLEAN, MUST CLEAN/SANITIZE; COOLERS, DISPLAY SHELVES, MEAT SLICER, ALL BURNERS, BOTTOM OF PREP TABLES AND OVENS.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. MUST RERAIR/SEAL FLOOR AT REAR UTENSILS WASHING AREA AND WALK IN COOLER AND WALK IN FREEZER, AND CLEAN FLOORS THRU-OUT PREMISES, INCLUDING BASEMENT, UNDER, BEHIND EQUIPMENT AT CORNERS AND ALONG WALLS. ALSO REPLACE MISSING AND BROKEN FLOOR TILES THRU-OUT.
The walls and ceilings shall be in good repair and easily cleaned. MUST REPAIR EAST WALL BEHIND OVEN AT REAR UTENSILS WASHING AREA, SEAL LARGE OPENING TO PREVENT PEST ENTRY, AND REPLACE DAMAGED CEILING PANELS.
Shielding to protect against broken glass falling into food shall be provided for all artificial lighting sources in preparation, service, and display facilities. MUST PROVIDE LIGHT SHIELDS FOR HOT HOLDING UNIT.
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. MUST PROPERLY INSTALL AN EXPOSED HAND WASHING BOWL WITH HOT/COLD WATER, SOAP AND PAPER TOWELS AT REAR UTENSIL WASHING AREA, SEAL RUSTY GREASE TRAP UNDER 3PART SINK AT REAR DISHWASHING AREA.
Soiled and clean linens, coats, and aprons shall be properly stored.
EXCESSIVE CLUTTER NOTED THRU-OUT PREMISES, INCLUDING BASEMENT, MUST REMOVE UNNECESSARY ARTICLES, CLEAN AND ORGANIZE TO PREVENT PEST HARBORAGE. ARTICLES MUST BE STORED OF FLOORS, AWAY FROM WALLS.
