Food handlers not wearing hair restraints. Must wear effective hair restraints.
Found food items not properly dated, labeled in walk in cooler, reach in freezers, reach in coolers, must date and label all food itemes in coolers, reach in freezers, walk in coolers, and reach in coolers. Must label all food containers(bins)in kitchen, rear prep area, rear storage areas,per code.
All necessary control measures shall be used to effectively minimize or eliminate the presence of rodents, roaches, and other vermin and insects on the premises of all food establishments, in food-transporting vehicles, and in vending machines. Found front entrance/exit door not rodent proofed, door had 1/4 inch on bottom of door.Also found/observed rodent droppings(30 ) in front storage area shelving by pots, pans, also found rodent droppings(50 ) in rear storage aread under and on storage shelving, pallets. Must provide pest control log book, and missing docummentation (IDPH license, pest sighting log, location of applied pesticides etc). Citation issued # H000063593, with a court date of 2/17/20/11, at 400 w superior in room #112 at 9:00am. Must seal opening at door, remove all droppings and wash/sanitize surfaces.
A separate and distinct offense shall be deemed to have been committed for each Minor violation that is not corrected upon re-inspection by the health authority.Previous minor violations not corrected from inspection report#150284, dated 1/26/09. Violation#32 must not use newspaper as liners for shelving in food prep areas.violation #35 white plastic walls throughout rear kitchen with dirty food splatter, must clean.
Exposed raw wood on shelving in food prep area shall be repair/replaced.Exposed raw wood shelving in rear storage areas shall be sealed with non toxic paint, or replaced.
Non food contact surfaces of cooler shelving, walk in cooler shelving/racks not clean need detailed cleaning(crevices). Non food contact surfaces of cooking equipment, stove, prep tables had excess grease build up, needs detailed cleaning(crevices). Must clean and maintain shelves/racks through-out display and storage areas.
Floors in kitchen prep area(rear) had excess grease, debris build up, needs detailed cleaning(corners). floors in front prep areas, rear storage areas under shelving, palletts need detailed cleaning (corners). Walk in cooler floor needs detailed cleaning. Remove all litter, dirt, debris etc.
Walls, ceiling in rear kitchen prep areas not clean needs detailed cleaning, walls in rear storage areas that had peeling paint gaods shall be repair.Ceiling tiles in front prep area need cleaning.
Missing light shields/end caps in rear storage and prep areas shall be provided, light shields in prep areas need cleaning.
Exhaust canopy not clean need detailed cleaning, exhaust vents(ventilation)in kitchen prep areas, coolers, walk in cooler not clean need cleaning. Must remove threaded hose adaptor from mop sink faucet.
Excess debris and clutter in rear storage areas shall be removed, stock items must be stored six inches off of floor to prevent pest harborage,

FOUND EVIDENCE OF MICE DROPPINGS IN THE REAR STORAGE AREA INSIDE THE SHELVINGS OVER 70 TOTAL AND IN THE FRONT WEST AND EAST CORNER OF THE STORE OVER 10 TOTAL. INSTRUCTED TO CLEAN AND SANITIZE AFFECTED AREAS. HAVE ADDITIONAL INSPECTION FROM PEST CONTROL COMPANY. CITATION ISSUED SERIOUS.
PREVIOUS MINOR VIOLATIONS NOT CORRECTED FROM REPORT #88648 DATED 1/19/11. #41- STOCK ITEMS MUST BE STORED 6 INCHES OFF THE FLOOR TO PREVENT PEST HARBORAGE. CITATION ISSUED SERIOUS.
MUST PROPERLY LABEL AND DATED ALL FOOD CONTAINERS INSIDE THE PREP COOLER, WALK IN COOLER, REACH IN COOLER AND REACH IN FREEZER. ALL PACKAGED FOODS FOR SALE MUST HAVE A LABEL, NAME OF THE COMPANY,ADDRESS AND PHONE, NAME OF THE PRODUCT. ALL BULK CONTAINERS WITH FOODS MUST HAVE A LABEL TOO.
MUST REPAIR CRACKED FLOOR TILES ON THE HALLWAY GOING TO THE KITCHEN. FLOORS UNDER AND BEHIND THE PREP COOLER IN FAST FOOD AREA, NEEDS DETAIL CLEANING AND MAINTAIN. CRACKED AND MISSING FLOOR TILES INSIDE THE WALK IN COOLER ,MUST REPAIR/REPLACE AND MAINTAIN. MUST CLEAN IN DETAIL THE FLOOR IN THE REAR PREP AREA ESPECIALLY AROUND THE GREASE BOX UNDER THE 3 COMPT SINK.
PEELING PAINT ALL OVER THE CEILING IN THE REAR STORAGE AREAS AND HALLWAY OF THE PREP AREA AND INSIDE THE WOMENS WASHROOM. MUST REMOVE PEELING PAINT OR REPAINT.
LEAKING ON THE FAUCET CONNECTOR OF THE 3 COMPT SINK. MUST REPAIR AND MAINTAIN. MUST REPAIR EXPOSED HAND SINK IN THE FAST FOOD AREA,(SLOW DRAINING).
MUST PROVIDE THERMOMETER AND MUST BE VISIBLE INSIDE THE REACH IN FREEZER.
MUST CLEAN IN DETAIL THE EXTERIOR SIDES AND TOP OF THE STOVES AND DEEP FRYER AND MAINTAIN. WIRE RACKS ABOVE THE EXPOSED HAND SINK IN THE REAR PREP AREA, WITH FOOD SPLATTER AND FOOD DEBRIS. MUST CLEAN IN DETAIL AND MAINTAIN.
