INSTRUCTED TO CLEAN WALLS AND CEILING INSIDE WALK IN COOLER, BY REAR EXIT DOOR, REPLACE WATER STAINED CEILING PANELS IN BASEMENT STORAGE AREA
INSTRUCTED TO CLEAN LIGHT SHIELDS THRU-OUT, PROVIDE LIGHT SHIELDS AT REAR PREP AREA, AND END CAPS AT FRONT AND REAR PREP

All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily. INSTRUCTED TO CLEAN AND MAINTAIN STORAGE RACKS AND CARTS WITH DRIED FOOD DEBRIS INSIDE WALK-IN COOLER.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. INSTRUCTED TO CLEAN FLOORS ALONG THE WALLS AND IN ALL CORNERS IN REAR BASEMENT STORAGE AREA.
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. INSTRUCTED TO REPAIR STRIPPED FAUCET (HOT) HANDLE AT MOPSINK IN REAR PREP AREA.
All parts of the food establishment and all parts of the property used in connection with the operation of the establishment shall be kept neat and clean and should not produce any offensive odors. INSTRUCTED TO REMOVE UNNECESSARY ARTICLES STORED IN BASEMENT STORAGE AREA TO PREVENT RODENT AND INSECT HARBORAGE.

Facilities not maintaining proper temperature. Internal temperature of cooler at 52.3 F while storing potentially hazardous foods. Instructed that all coolers must maintain 40 F or below. 'Held for Inspection' tag placed on cooler. Critical citation issued 7-38-005(A).
Potentially hazardous foods does not meet temperature requirement during storage, preparation, and service. Found 1 gallon of milk at 49.7 F, 10 lbs of Provolone Cheese at 58.8 F. Instructed that potentially hazardous foods must maintain 40 F or below. Manager voluntarily denatured and discarded. Value $45, 18 lbs. Critical citation issued 7-38-005(A).
Dish washing facilities not maintained or operated. While running and draining water at both front and rear three compartment sinks, water coming up both left and right sides. Instructed to detail clean grease trap and drainline to prevent inadequate disposal of waste water. Also, front three compartment sink faucet does not reach all three compartments to properly wash and sanitize utensils. Instructed to replace faucet. Serious citation issued 7-38-030.
