Shielding to protect against broken glass falling into food shall be provided for all artificial lighting sources in preparation, service, and display facilities.CLEAN LIGHT SHIELD IN LUNCH ROOM,OBSERVED SPLATTERED FOOD ON SHIELD,AND DUST BUILD UP.

The walls and ceilings shall be in good repair and easily cleaned. PEELING PAINT AT UTILITY MOP SINK, MUST REMOVE/REPAIR. CEILING ABOVE THREE COMPARTMENT SINK IN POOR REPAIR WITH CHIPPING DAMAGED PLASTER, MUST REPAIR.
Containers and utensils shall be inverted, covered, or otherwise protected from contamination until used. All single-service drinking straws and containers shall be discarded immediately after use. TABLE TOP CAN OPENER WITH ENCRUSTED FOOD DEBRIS. MUST MAINTAIN.
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. MUST REMOVE THREADED NOZZLE FROM KITCHEN EXPOSED HAND SINK. ENGINEER REMOVED.

All food shall be protected from contamination and the elements, and so shall all food equipment, containers, utensils, food contact surfaces and devices, and vehicles. OBSERVED DURING INSPECTION EMPLOYEE CLEANING EQUIPMENT IN SINK AND STOVE WITH DIRTY CLEANING CLOTH THEN STOP TO WRAP CHEESE ON PREP TABLE IN PLASTIC WRAP, STORE CHEESE IN REACH-IN COOLER THEN CONTINUE TO RETURN TO CLEANING DUTIES CONTAMINATING THE CHEESE BY NOT WASHING HANDS PRIOR TO FOOD HANDLING. APPROXIMATELY 3LBS CHEESE DISCARDED. REVIEWED PROPER METHOD OF FOOD HANDLING WITH CERTIFIED MANAGERS.
