Violation #18 has been abated. Rear screen door was removed. Instructed manager to close the exit door for the mean time. Per manager he order a new screen door and will install it when he got it.

Cooler shelving that had peeling paint, rust shall be repair/replaced, front counter in prep area had exposedraw wood, shall be repair/sealed. All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair.
Non food contact surfaces of cooking equipment, cooler shelving, prep tables, and storage shelving not clean need detailed cleaning(crevices). All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily.
Floors under heavy equipment, cooking equipment, storage shelving, 3 compartment sink, and storage shelving not clean need detailed cleaning(corners).
Walls that hag gaps, openings in rear kitchen prep area, shall be repair, damaged, stained ceiling tiles in kitchen prep areas shall be replaced/repair,ceiling in front prep areas not clean need detailed cleaning. The walls and ceilings shall be in good repair and easily cleaned.
Missing light shields in rear kitchen prep area shall be provided, light shields in kitchen prep areas not clean need detailed cleaning, light shields in coolers need cleaning.
Exhaust canopy had excess greae build up need detailed cleaning, exhaust vents(ventilation) in coolers, kitchen areas not clean need detailed cleaning.

Found observed rear exit door not rodent proofed, door had 1/4 inch gaps on bottom, and side of door, shall be repair/replaced, also found broken window in prep area(outer openings not protected), shall be repair/replaced.Citation#H000072739 with a court date of 12/15/2011, at 400 W Superior in Rooom #112 at 2:00 PM.
