The area outside of the establishment used for the storage of garbage shall be clean at all times and shall not constitute a nuisance. OBSERVED 2 DUMPSTERS MISSING LIDS AND 3 DUMPSTERS WITH DAMAGED LIDS. INSTRUCTED MANAGER TO HAVE ALL MISSING AND DAMAGED LIDS REPLACED TO RODENT PROOF DUMPSTERS.
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. All necessary control measures shall be used to effectively minimize or eliminate the presence of rodents, roaches, and other vermin and insects on the premises of all food establishments, in food-transporting vehicles, and in vending machines. found/observed entrance/exit doors not rodent proof in rear kitchen, door had 1/4inch gap(middle of door)shall be repair. doors in receiving area, 2nd floor, and all exterior door throught building that were not rodent, insect proofed, and had gaps on bottom of doors ranging from 1/4 inch, to 1/2 inch shall repair. citation issued H000063577, with a court date of 12/09/2010, at 400 w superior in room #112 at 9:00am.
