Found leaky drainage piping at preparation sink at rear of premises.(leaky water release handle).
All employees shall be required to use effective hair restraints to confine hair. Found/observed foodhandlers not wearing hair restraints.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. Found floors not clean by 3compartmnet sink area.

All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. PREP AREA REACH IN SALAD COOLER DOOR RUBBER GASKETS RIPPED. MUST REPAIR AND MAINTAIN DAMAGED DOOR GASKETS.
Between uses and during storage ice dispensing utensils and ice receptacles shall be stored in a way that protects them from contamination. OBSERVED CRACKED PLASTIC ICE SCOOP INSIDE ICE MACHINE. INSTRUCTED MANAGER TO REPLACE DAMAGED ICE SCOOP.
