REMOVE UNUSED ITEMS AND EQUIPMENT FROM 1ST. FLOOR AND BASEMENT STORAGE AREA AND REMOVE MILK CRATES BEING USED TO STORE ITEMS AND REPLACE WITH RACKS TO ELEVATE 6 INCHS OFF FLOOR AND AWAY FROM WALL.
All cold food shall be stored at a temperature of 40F or less. FOUND POTENTIALLY HAZARDOUS FOODS AT IMPROPER TEMPERATURE: 15 LBS OF COOKED CHICKEN AND 30LBS OF COOKED BEANS INSIDE WALK-IN COOLER AT 63.5F. MANAGER STATES PRODUCT WAS PLACED INTO UNIT ON 07/05/10. NO TIME/TEMPERATURE LOGS PROVIDED. MANAGER VOLUNTARILY DISCARDED SAID FOOD ITEMS, APPROXIMATE VALUE $50.00. INSTRUCTED TO COOL HOT FOODS FROM 140F TO 70F IN TWO HOURS AND 70F TO 40F IN FOUR HOURS. CRITICAL 7-38-005(A) ISSUED.
All food not stored in the original container shall be stored in properly labeled containers. LABEL BULK FOOD CONTAINERS, FROZEN PACKAGED FOODS, AND READY TO EAT, PREPARED FOODS INSIDE WALK-IN COOLER AND FREEZER.

The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. CLEAN FLOORS IN BASEMENT AND REAR 1ST. FLOOR BY WALK-IN COOLER TO REMOVE DEBRIS. REMOVE STANDING WATER SECTIONS IN BASEMENT, ESPECIALLY BY HOT WATER TANK.
The walls and ceilings shall be in good repair and easily cleaned. CLEAN WALL SECTION BY UTILITY SINK, REMOVE BUILDUP AT WALL BASE BY UTILITY SINK, CLEAN CEILING AND VENTS IN KITCHEN PREP AREA, SEAL OPENINGS AT PIPING AND CONDUITS AT REAR STORAGE/UTILITY SINK AREA.
REPAIR LEAKS AT HANDLES AT THREE COMPARTMENT SINK AND PROVIDE HOT WATER HANDLE AT UTILITY SINK. INSTALL DIPPER WELL WITH RUNNING COLD WATER FOR HAND DIPPED ICE CREAM SCOOPS.

All parts of the food establishment and all parts of the property used in connection with the operation of the establishment shall be kept neat and clean and should not produce any offensive odors. FOUND SOME CLUTTER IN THE BASEMENT. INSTRUCTED TO CLEAN AND ORGNIZE SO NO INSECT OR RODENT HARBORAGE.
All necessary control measures shall be used to effectively minimize or eliminate the presence of rodents, roaches, and other vermin and insects on the premises of all food establishments, in food-transporting vehicles, and in vending machines. FRONT DOOR AND BACK DOOR HAS A ONE INCH GAP AT THE BOTTOM IF THEM. ALSO FOUND 25-30 MICE DROPPINGS IN THE STORE ROOM BY SOUTH WALL. INSTRUCTED TO CLEAN UP THE AREA, MAKE BOTH DOORS TIGHT FITTING AND RODENT PROOFED. ALSO CALL YOUR PEST CONTROL COMPANY TECH TO COME HERE TO SERVICE THIS ESTABLISHMENT. All utensils shall be thoroughly cleaned and sanitized after each usage. CLEAN ALL THE COOKING EQUIPMENT, HOOD AND FILTERS ABOVE HAS GREASE BUILDUP.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. CLEAN THE FLOORS IN THE BACK STORE ROOM,PREP AREA AND IN THE BASEMENT HAS DUST,DIRT AND FOOD DEBRIS IN THEM.
