The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. Found floors not clean at basement.
Shielding to protect against broken glass falling into food shall be provided for all artificial lighting sources in preparation, service, and display facilities. Found broken light shields at rear preparation areas.
All employees shall be required to use effective hair restraints to confine hair. Observed foodhandler not wearing hair restraint.
Found exterior of 3compartment sink not clean.
All food establishments that display, prepare, or store potentially hazardous food shall have adequate refrigerated food storage facilities. FOUND STAND UP REACH IN FREEZER NOT MAINTAINED, AMBIENT AIR TEMPERATURE AT 52.0 F. FOUND CHICKEN, BEEF, ETC INSIDE COOLER. INSTRUCTED MANAGER TO REPAIR AND MAINTAIN FREEZER.
All cold food shall be stored at a temperature of 40F or less. FOUND THE FOLLOWING POTENTIALLY HAZARDOUS FOODS HELD AT IMPROPER TEMPERATURES BETWEEN 47.4 AND 52.0 F- BEEF BURGER PATTIES, CHICKEN WINGS, PIZZA PUFF, MOZZARELLA CHEESE, BEEF GRAVY, PEARCH, CATFISH, GYROS, TALPIA. INSTRUCTED MANAGER ALL POTENTIALLY HAZARDOUS FOODS MUST BE HELD AT 40 F OR BELOW. ALL OUT OF TEMPERATURE POTENTIALLY HAZARDOUS FOODS VOLUNTARILY DISCARDED AT THIS TIME. WEIGHT OF FOOD DISCARDED 150 LBS, VALUED AT OVER $200.00
All food should be properly protected from contamination during storage, preparation, display, service, and transportation. OBSERVED LARGE HOLE IN CEILING ABOVE PREP AREA TABLE WITH WATER DRIPPING FROM HOLE ALSO OBSERVED LARGE HOLE IN CEILING ABOVE WASHROOM. INSTRUCTED MANAGER TO NOT PREPARE FOOD AT PREP TABLE UNDER LEAK FROM CEILING, ALSO MUST REPAIR/SEAL BOTH LARGE HOLES IN CEILING BY DATE OF REINSPECTION (09/06/11).
