The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. MUST DETAIL CLEAN FLOORS UNDER 3 COMPARTMENT SINKS,UNDER PREP TALES AND UNDER ICE MACHINE.
All food not stored in the original container shall be stored in properly labeled containers. LABEL ALL BULK FOOD ITEM CONTAINERS.
The flow of air discharged from kitchen fans shall always be through a duct to a point above the roofline. MUST REPAIR LEAKY FAUCET AT 3 COMPARTMENT SINK IN REAR. REPAIR LEAK UNDER DISH MACHINE.

All food should be properly protected from contamination during storage, preparation, display, service, and transportation. FOOD (ICE) NOT PROTECTED FROM CONTAMINATION INSIDE OF THE ICE MACHINE. OBSERVED MOLD GROWTH ON INTERIOR CONTACT SURFACES OF THE ICE MACHINE. THE INTERIOR OF THE MACHINE WAS CLEANED AND SANITIZED DURING THE INSPECTION. SERIOUS VIOLATION 7-38-020.
All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. REPLACE THE SHELVING ABOVE THE PREP COOLER AT THE COOK'S LINE.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. REPAIR THE FLOOR IN THE EMPLOYEE RESTROOM SO THAT IT IS SMOOTH AND EASILY CLEANABLE.
The walls and ceilings shall be in good repair and easily cleaned. DETAIL CLEAN THE WALL BEHIND THE SUSHI PREP COUNTER. REPLACE THE WATER STAINED CEILING TILES IN THE MEN'S WASHROOM.
Shielding to protect against broken glass falling into food shall be provided for all artificial lighting sources in preparation, service, and display facilities. MUST PROVIDE A LIGHT SHIELD FOR THE LIGHT FIXTURE ABOVE THE COOKING EQUIPMENT ALONG THE COOK'S LINE VENTILATION HOOD.
All food establishments that display, prepare, or store potentially hazardous foods shall have calibrated metal stem thermometers, provided and conspicuous, for refrigerated and hot food units. THERMOMETERS ARE NEEDED FOR ALL REFRIGERATION UNITS. (SUSHI COOLER, REAR PREP COOLERS)
The area outside of the establishment used for the storage of garbage shall be clean at all times and shall not constitute a nuisance. OBSERVED GARBAGE OVERFLOWING FROM THE GARBAGE DUMPSTER IN THE BACK OF THE ESTABLISHMENT THAT WAS MARKED WITH THE NAME AND ADDRESS OF THE RESTAURANT. MANAGEMENT INSTRUCTED THAT THE GARBAGE AREA MUST BE CLEAN AT ALL TIMES AND THAT ADEQUATE PICKUP MUST BE MAINTAINED TO PREVENT A NUISANCE AND A PEST ISSUE. SERIOUS VIOLATION 7-38-020.
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. SINK STOPPERS ARE NEEDED FOR THE REAR PREP 3 COMPARTMENT SINK. MUST PROVIDE.
