All food should be properly protected from contamination during storage, preparation, display, service, and transportation.OBSERVED BLACK SLIMY SUBSTANCE BUILT UP INSIDE ON INTERIOR PARTS OF ICE MACHINE AND DRIPPING ONTO ICE THAT IS BAGGED ON SITE FOR SALE.A HELD FOR INSPECTION TAG PLACED ON UNIT .MUST DETAIL CLEAN & SANITIZE ICE MACHINE.
All necessary control measures shall be used to effectively minimize or eliminate the presence of rodents, roaches, and other vermin and insects on the premises of all food establishments, in food-transporting vehicles, and in vending machines.OBSERVED EVIDENCE OF NUMEROUS OPEN & BAGS OF POISON SCATTERED ON FLOOR UNDER CHIP DISPLAY UNITS & UNDER UNUSED SLUSHY MACINE,IN BETWEEN CRATES IN REAR STORAGE AREA,6-8 LIVE HOUSE FLIES ON SITE.ALSO FRONT & REAR DOORS NOT TIGHT FITTING.MUST REMOVE POISON,CLEAN & SANITIZE AREAS,RODENT PROOF DOORS.RECOMMENDED TO HAVE A PEST CONTROL OPERATOR TO SERVICE PREMISES ON OR AFTER 6/29/11.
