All food not stored in the original container shall be stored in properly labeled containers. MUST LABEL BULK SUGAR CONTAINER.
The walls and ceilings shall be in good repair and easily cleaned. HOLE IN WALL AND CEILING OF CUSTOMER MENS ROOM. MISSING WALL TILE OF CUSTOMER LADIES WASHROOM. MUST REPAIR ALL. CHIPPING PLASTER ON CEILING OF DRY STORAGE ROOM. MUST REPAIR.
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. LEAKS AT BOTH NOZZLES ON THREE COMPARTMENT SINK. MUST REPAIR.
All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily. EXTERIOR DISH MACHINE MUST CLEAN.

MUST STORE WET WIPING CLOTHS IN SANITIZER INSTEAD OF THE TOPS OF THE PREP TABLES AND PREP COOLERS AND COOKING EQUIPMENT.
All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. OBSERVED KNIFE STORED IMPROPERLY UNDERNEATH THE SHAKE MIXER. MANAGEMENT INSTRUCTED TO PROVIDE A KNIFE RACK AT THE COOK'S LINE.

PAINT AREAS OF PEELING PAINT AND FILL IN OPENINGS IN THE WALLS. REGLUE THE VINYL BASEBOARD IN THE DRY STORAGE AREA NEXT TO THE COOK'S LINE. REPAINT THE RUSTED AND CHIPPED PAINT ON THE VENT COVER IN THE DISHWASH AREA.
