All necessary control measures shall be used to effectively minimize or eliminate the presence of rodents, roaches, and other vermin and insects on the premises of all food establishments, in food-transporting vehicles, and in vending machines. OUTER OPENINGS ARE NOT RODENT PROOFED. OBSERVED A 3/4' GAP ALONG THE BOTTOM OF THE FRONT DOOR. MANAGEMENT INSTRUCTED TO RODENT PROOF THE FRONT DOOR. SERIOUS VIOLATION 7-38-020.
A certified food service manager must be present in all establishments at which potentially hazardous food is prepared or served. NO CERTIFIED FOOD MANAGER PRESENT WHILE POTENTIALLY HAZARDOUS FOODS (FALAFEL, LAMB, HUMMUS, ETC.) ARE BEING PREPARED OR SERVED. MANAGEMENT INSTRUCTED THAT A CERTIFIED FOOD MANAGER MUST BE PRESENT AT ALL TIMES WHILE POTENTIALLY HAZARDOUS FOOD IS BEING PREPARED OR SERVED. SERIOUS VIOLATION 7-38-012.
Containers and utensils shall be inverted, covered, or otherwise protected from contamination until used. All single-service drinking straws and containers shall be discarded immediately after use. MUST INVERT THE TAKE-OUT STORAGE CONTAINERS STORED ON THE SHELF SO THAT THEY ARE PROTECTED FROM CONTAMINATION.
All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily. DETAIL CLEAN THE VENTILATION HOOD AND FILTERS, MEAT SLICER, PREP TABLE BELOW THE GYROS MACHINE, TOASTER OVEN AND MICROWAVE OVEN.
All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. MUST REPLACE THE SPRAY NOZZLE AT THE 3 COMPARTMENT SINK. THE RIGHT DOOR FOR THE PREP COOLER IS FALLING OFF OF THE HINGES. MUST REPAIR.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. DETAIL CLEAN THE FLOOR THROUGHOUT THE PREP AREA ESPECIALLY UNDER THE COOKING EQUIPMENT. REPLACE THE BROKEN AND MISSING FLOOR TILES OUTSIDE OF THE UTILITY CLOSET AND IN THE RESTROOM.
The walls and ceilings shall be in good repair and easily cleaned. DETAIL CLEAN THE WALLS IN THE PREP AREA, ESPECIALLY AROUND THE SINKS AND BEHIND THE COOKING EQUIPMENT.
All rooms in which food or drink is prepared, or in which utensils are washed, shall be lighted so that a minimum of 50-foot candles of light is available on all work surfaces. MUST PROVIDE ADDITIONAL LIGHTING IN THE PREP AREA.
All food establishments that display, prepare, or store potentially hazardous foods shall have calibrated metal stem thermometers, provided and conspicuous, for refrigerated and hot food units. MUST PROVIDE A METAL STEM THERMOMETER AND THERMOMETERS INSIDE OF ALL COOLERS.

NO CITY OF CHICAGO CERTIFIED MANAGER PRESENT DURING INSPECTION WHILE POTENTIALLY HAZARDOUS FOODS WERE BEING PREPARED AND SERVED. MANAGER WAS TELEPHONED AT INITIAL INSPECTION AND ARRIVED AT 12:30 PM.SERIOUS CITATION ISSUED.
MUST REMOVE OLD BROKEN TOILET FROM REAR PATIO. EMPLOYEES MUST NOT STORE PERSONAL BELONGING/CLOTHING IN REAR DRY FOOD STORAGE ROOM.
BROKEN VENTILATION IN EMPLOYEE WASHROOM. MUST REPAIR.
ALL PREPARED FOODS IN ALL COOLERS MUST BE LABELED AND DATED. ALL BULK FOOD CONTAINERS MUST BE LABELED.
WIPING CLOTHS MUST BE STORED IN A CONTAINER WITH A SANITIZING SOLUTION.
ALL EMPLOYEES IN FOOD PREP AREA MUST WEAR HAT/HAIR RESTRAINT.
