Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health.exhaust canopy not clean needs detailed cleaning, exhaust vents(ventilation) in coolers, kitchen prep areas, and bathrooms need detailed cleaning.
All food not stored in the original container shall be stored in properly labeled containers.found some food items not properly dated, labeled in coolers, must date and label all food items in coolers.
All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair.non food contact surfaces of cooler shelving that had rust, shall be repair/replaced.

OBSERVED MOP SINK WATER VALVE LEAKING AND 3-COMPARTMENT SINK HOT WATER HANDLE AND FAUCET LEAKING (AT BASE). MUST REPAIR AND MAINTAIN SINKS.
All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. OBSERVED DARK PITS ON SURFACE OF CUTTING BOARDS IN PREP AREA ABOVE REACH IN COOLERS. MUST REPAIR/REPLACE DAMAGED CUTTING BOARDS.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. OBSERVED GAP IN FLOOR BASE UNDER WOK. MUST SEAL GAP IN WALL TO PREVENT POTENTIAL PEST HARBORAGE.

OBSERVED DAMAGED/WATER STAINED CEILING TILES ABOVE PREP AREA. MUST REPLACE ALL DAMAGED CEILING TILES.
