A separate and distinct offense shall be deemed to have been committed for each Minor violation that is not corrected upon re-inspection by the health authority. Previous minors from 2/25/09 visit, not corrected at this time. (#32-cutting boards w/ deep/dark grooves must be replaced; #33-air vents of display cases thruout, cabinets under soda syrup racks at soda fountain need cleaning due to dust/debris build-up; #34-floors under shelvings at milk/beer coolers must be kept clean; #35-dusty fan guards/condensers of coolers thruout premises must be cleaned & missing wall panel at bottom of 3-comp sink in rear including any holes or openings around pipes or cords on walls or ceiling must be sealed. Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. Must fix/provide ventilation in bathrooms; fix leaky faucets at 3-comp sinks (x2). All parts of the food establishment and all parts of the property used in connection with the operation of the establishment shall be kept neat and clean and should not produce any offensive odors. Any milk/soda crates used for storage shelvings must be removed & replaced with regular shelves/tacke that ca be accessible to cleanig/pest monitoring. The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. Floors under dry storage shelvings in rear hallway to butcher cooler must be free of clutter or debris.
