Toilet rooms near classrooms for students male and female not maintained or operated. Handsinks in toilet rooms and utility sink next to toilet room not provided with hot running water for proper hand washing for students. Instructed that all handsinks must supply hot and cold running water under city pressure at all times. Serious citation issued 7-38-030.
Storage cabinets and sinks in prep area not cleaned. Instructed to detail clean and sanitize.
Floors in corners and in boiler room along wallbase not cleaned. Instructed to detail clean.
Windows in toilet room not protected. Instructed to install 16inch mesh screen over windows. Holes noted around pipes in kitchen area and fixtures in toilet room. Instructed to seal all openings.
Faucet not maintained on utility sink. Instructed to repair faucet to prevent leak.
Thermometer not provided inside refrigator in kitchen. Instructed to provide thermometer visible inside cooler.

Found previous minor violation #35 from inspection reports#250206,and 250496, dated 5/24/2010,and 6/02/2010, with a correction date of 8/24/2010 for both reports, not correct at this time of inspection on 9/08/2011.minor violation#35 stated: Windows in toilet room not protected. instructed to install 16 inch mesh screen over windows.Citation #H000072732 issued with a court date of 10/06/2011, at 400 W Superior in room #112 at 2:00 PM.
Non food contact surfaces of reach in cooler shelving/racks not clean need detailed cleaning, non food contact surfaces of 3 compartment sink not clean need deatiled cleaning.
Floors inkitchen area under 3 compartment sink, heavy equipment not clean need detailed cleaning(corners).
Peeling paint in clasroom ceiling, walls in 2nd floor shall be repair/sealed, damaged/stained ceiling tiles shall be replaced in hallways(2nd floor).
Missing light shields in girls bathroom shall be provided, light shields in bathrooms, teachers lounge not clean need cleaning.
