FRONT ENTRANCE DOOR NOT RODENT PROOFED AT BOTTOM OR SIDE; MUST RODENT PROOF DOOR. OBSERVED LIVE FRUIT FLIES AT PRODUCE COOLER; MUST ELIMINATE ACTIVITY.
All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. OBSERVED METAL STORAGE PLATES IN PRODUCE COOLER RUSTY; INSTRUCTED TO PAINT OR REPLACE. OWNER STATES SHE IS GOING TO PURCHASE A SMALLER PRODUCE COOLER.
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. MUST INSTALL A UTILITY SINK ON PREMISES FOR DISPOSAL OF WASTE WATER. SINK MUST HAVE A BACKFLOW DEVICE ATTACHED TO THE WATER LINE. HANDSINK IN WASHROOM HAS NO COLD RUNNING WATER; MUST PROVIDE. ALSO SINK LEAKS FROM AREA WHERE KNOBS ATTACHED WHEN HOT WATER RUNS; REPAIR.
