All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily. May 22, 2012Found observed food items not properly dated/labeled in walk in cooler, reach in cooler(1st floor), must date and label food items in walk in cooler, reach in cooler at all times.
Reach in cooler shelving/racks in front prep area had peeling paint/rust, shall be repaired/sealed, caulk around prep table in front prep area in state of disrepair, shall be repaired/replaced.
Non food contact surfaces of walk in cooler shelving/racks not clean, need detailed cleaning, non food contact surfaces of ice machine not clean(1st floor), need cleaning(detailed), rear kitchen storage shelving/racks not clean need cleaning, non food contact surfaces storage cabinet shelving(16th floor)need cleaning.
Floors under heavy equipment, cooking equipment in kitchen prep area not clean, need detailed cleaning, floors throughout kitchen prep area, and along wallbase, had missing mortar, standing water, shall be repaired sealed, and re-grouted, to prevent standing water in kitchen prep areas (1st floor).floors in men's bathroom(1st floor need cleaning).
Ceiling tiles in kitchen prep areas that were damaged/stained, shall be replaced, ceiling tiles in kitchen prep areas not clean need detailed cleaning, ceiling tiles in receiving areas, cafeteria need cleaning, damaged ceiling tiles shall be replaced(bsmt).

Found/observed employees not washing their hands while changing gloves, handlind foods(beef, cutting vegetables), and preparing foods at time of inspection, citation issued.Mgmt. was instructed in proper procedures for hand washing while preparing, handling foods, including ready to eat foods.
Found observed front entrance door not rodent proofed, door had 1/4inch gap on middle of door, also found exit doors in boiler room not rodent proofed, doors had gaps ranging from 1/4 inch to 1/2 inch, Citation issued.
