Live flies-house and gnats through premises, approximately 10 to 15 plus.Front entry door not rodent proofed and also broken-will not close. Front door is made of plexi-glass, glass cracked and missing from door frame. Pest control log book incomplete on premises. Instructed to remove and detail clean all areas of rodent activity. Must repair or replace door, to have door tight fitting and rodent proofed to prevent rodent and insect entry. Must have a completed pest control log book with all required written documents of services performed per Rule C. Serious citation issued 7-38-020. Hand washing facilities not provided with hand drying device. Employees toilet room not provided with hand drying device or disposal hand toilet for proper hand washing of employees. Instructed to replace broken sprayer at faucet and repair over head pipes to prevent leaks. Thermometers not provided inside coolers. Instructed to provided hanging thermometers visible inside all coolers. Unnecessary articles stored throught premises. Instructed to elevate stock and supplies 6 inches or higher and away from wall and remove any unused articles and detail clean. Light not provided in storage room. Instructed to provide adequate light and/or replace bulbs. Toilet room door not self-closing. Instructed to install self-closing device. Floors not maintained through-out. Instructed to replace any missing floor tile and must detail clean under all display shelves and in storage areas. Shelves inside coolers, display shelves and freezer rusted. Instructed to remove rust and reseal with non-toxic sealant or replace equipment. Storage shelves, chest freezer, display shelves and sinks not cleaned. Instructed to detail clean and sanitize.
