A separate and distinct offense shall be deemed to have been committed for each Serious violation that is not corrected upon re-inspection by the health authority. #21- CONTINUED NON-COMPLIANCE OF NO CITY OF CHICAGO CERTIFIED MANAGER PRESENT DURING INSPECTION WHILE FOODS ARE BEING PREPARED AND SERVED. LAST ROUTINE INSPECTION ON 1-28-09 #154902 LICENSEE WAS SITED 7-38-012, NO CERTIFIED MANAGER ON SITE AND DURING THIS INSPECTION NO CERTIFIED MANAGER WAS PRESENT DURING THE INSPECTION. OUTSIDE WASTE RECEPTACLES OVERFLOWING WITH BAGS OF FOOD DEBRIS, GARBAGE, CARDBOARD BOXES. LIDS UNABLE TO CLOSE ON BOTH CONTAINERS. ALSO PLASTIC SODA BOTTLES AND BAGS ON GROUND SURROUNDING CONTAINERS. INSTRUCTED TO REMOVE ALL DEBRIS FROM GROUND, REMOVE OVERFLOWING GARBAGE FROM CONTAINERS AND RECOMMEND MORE FREQUENT GARBAGE PICK UP OR ADDITIONAL CONTAINERS.
All food establishments that display, prepare, or store potentially hazardous foods shall have calibrated metal stem thermometers, provided and conspicuous, for refrigerated and hot food units. REPLACE INACCURATE THERMOMETER IN WALK IN COOLER.
Between uses and during storage ice dispensing utensils and ice receptacles shall be stored in a way that protects them from contamination. ALL WIPING CLOTHS MUST BE STORED IN A CLEAN CONTAINER WITH A SANITIZING SOLUTION.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. MUST NOT USE CARDBOARD AS MATTING FOR FLOORS.
