The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. CLEAN FLOORS UNDER, AROUND AND BEHIND KITCHEN EQUIPMENT AND EQUIPMENT IN STORAGE ROOM.
All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. REMOVE DUST FROM REAR AREA OF STOVE AND MAINTAIN.
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. TOILET IN BOYS WASHROOM ON FIRST FLOOR WAS CLOGGED WITH TOILET PAPER; INSTRUCTED TO UNCLOG. WASHROOM HAS URINALS AND AN ADDITIONAL BOYS WASHROOM IS IN BASEMENT.

All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair.OBSERVED 3 TIER TABLE INSTRUCTED TO REMOVE RUST. ALSO NEED TO REMOVE ALL CONTACT PAPER ON BOTTOM OF THE PREP TABLE.
All utensils shall be thoroughly cleaned and sanitized after each usage. NEED TO CLEAN INTERIOR AND EXTERIOR OF ALL CABINETS,CLEAN AND SANITIZE COUNTERS.
The walls and ceilings shall be in good repair and easily cleaned.CLEAN WALLS THRU OUT KITCHEN.
Shielding to protect against broken glass falling into food shall be provided for all artificial lighting sources in preparation, service, and display facilities.NEED TO CLEAN LIGHT SHIELDS

MUST SEAL RAW WOOD STORAGE SHELVES IN KITCHEN AND DRY STORAGE SHEVES.
MUST INSTALL BACK FLOW DEVICE ON MOP SINK FOR WATER HOSE USE.
