Smoking shall be prohibited in all public places except in designated smoking areas. A MAN WAS OBSERVED SMOKING IN THE OUTDOOR PATIO SEATING AREA AND A LARGE ASHTRAY WAS FOUND IN SAID AREA. SMOKING IS NOT ALLOWED IN YOUR OUTDOOR PATIO AREA.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. THE FLOORS AREW DIRTY UNDER AND BEHIND THE EQUIPMENT, Clean and maintain the floors and baseboards so they are smooth and sanitary from corner to corner.
The walls and ceilings shall be in good repair and easily cleaned. Clean and maintain the walls and ceilings so they are smooth and sanitary from top to bottom.
All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. THE KITCHEN TABLES AND SHELVES MUST BE WASHED, RINSED AND SANITIZED.

Repair the leak under the dish sprayer.
All necessary control measures shall be used to effectively minimize or eliminate the presence of rodents, roaches, and other vermin and insects on the premises of all food establishments. 10-15 live cockroaches were found behind the grease trap and under the pantry cooler on the floor. 8 smallflies were found on a carved pumpkin in the bar. Remove all evidence of insect activity. Pest proof the south outer door into the kitchen.
All food should be properly protected from contamination during storage, preparation, display, service, and transportation. The bar service station ice scoop is stored in a filty plastic container in brackish liquid, the soda dispensing guns are stored in rubber holsters full of slimey buildup. Wash, rinse and sanitize all of the food dispensing utensils and equipment until it is clean, sanitary and maintained on a daily basis.
All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. All food shelving and surfaces in the kitchen and bar are dirty. Clean, rinse and sanitize all buildup and degrease the equipment and surfaces in detail from top to bottom. Remove encrusted food buildup from under and around the dish machine and delime the dishmachine interior.
All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily. All utensils shall be thoroughly cleaned and sanitized after each usage. Move away from the walls every piece of kitchen equipment and remove all of the loose debris, food and grease buildup until they are clean and sanitary.

All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily. THE SAUTE STATION COOLER IS BROKEN, REPAIR OR REPLACE THE COOLER. REMOVE THE ROTTEN WOOD BELOW THE BAR ICE BIN.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. THE FLOOR UNDER FRYERS IS GREASY, CLEAN THE FLOOR IN DETAIL.
The walls and ceilings shall be in good repair and easily cleaned. SEAL THE GAPS IN THE WALL ABOVE THE HOT LINE PASS SO IT IS SMOOTH AND EASY TO MAINTAIN.
ONE OF THE WOMEN'S ROOM SINKS IS OUT OF ORDER. REPAIR OR REPLACE THE SINK.
