All necessary control measures shall be used to effectively minimize or eliminate the presence of rodents, roaches, and other vermin and insects on the premises of all food establishments, in food-transporting vehicles, and in vending machines.Found/observed rear exit door leading to garbage containers not rodent proofed, doors had 1/4 inch gap on bottom of door, shall be repair by 1/21/2011, Citation issued#H000063594, with a court date of 2/17/2011, at 400 w superior in room #112 at 9:00am.
Exposed raw wood in rear storage shelving, shall be repair/replaced. All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair.
Non food contact surfaces of cooler shelving, ice machine, prep table lower shelving in kitchen areas need cleaning.Lower shelving of pop dispensing cabinet not clean needs detailed cleaning.
Floors umder heavy equipment, 3 compartment sink, rear storage shelving not clean need detailed cleaning(corners).
Walls, ceiling tiles in rear kitchen prep area not clean need detailed cleaning.
Light shield in rear kitchen prep area not clean need detailed cleaning, light shields in bathroom need cleaning.
Exhaust vents(ventilation) in prep areas, above oven, rear storage areas, walk in cooler not clean need cleaning(detailed).

Found some food items not properly dated/labeled in walk in freezer, must date and label food items in walk in freezer.
Non food contact surfaces of pop dispensing unit nozzles not clean need detailed cleaning.
Rear ceiling above ice machine had peeling paint, shall be repaired/sealed, exposed raw wood in rear kitchen area shall be sealed.

OBSERVED MALE EMPLOYEE COMING FROM THE FRONT DOOR TO START HIS SHIFT WHILE WAITING FOR THE CUSTOMER AND ONE FEMALE EMPLOYEE WAITING FOR THE CUSTOMER ALSO, STARTED PUTTING THE GLOVES ON TO GET THE ORDER FOR THE CUSTOMER WITHOUT WASHING THEIR HANDS AND STARTED PREPARING THE SANDWICHES. INSTRUCTED MANAGER AND EMPLOYEES THAT ALL EMPLOYEES MUST WASH THEIR HANDS BEFORE PUTTING THE GLOVES ON AND AFTER REMOVING THE GLOVES, BEFORE PREPARING SANDWICHES AND AS NECESSARY AS NEEDED. CITATION ISSUED CRITICAL.7-38-010A.
MUST INSTALL A SPLASHGUARD BETWEEN THE PREP TABLE AND THE EXPOSED HAND SINK IN THE REAR PREP AREA(LOCATED TOO CLOSE TO EACH OTHER).
MISSING LIGHT SHIELD ON ONE OF THE LIGHT BULB IN THE FRONT SERVING AREA. MUST PROVIDE LIGHT SHIELD.
MUST REMOVE THE THREAD OF THE MOP SINK FAUCET OR INSTALL A BACKFLOW PREVENTION DEVICE. SLOW DRAINING OF TWO OF THE SINKS OF THE 3 COMPARTMENT SINK. MUST CLEAN THE GREASE TRAP BOX AND MAINTAIN.

MANAGER IMMEDIATELY AND VOLUNTARILY DISPOSED OF 12IBS OF FOOD WORTH $45 THROUGH DENATURING PROCESS. INSTRUCTED OF THE IMPORTANCE OF TEMPERAURE MONITORING. (TEMPERAURE OF SERVING LINE WAS 40F). MANAGER STATES REMOVED FOOD ITEMS TO CLEAN AND MAINTAIN SERVING LINE SINCE THEY ARE OPEN 24HOURS. CRITICAL VIOLATION ISSUED FOR VIOLATION #7-83-005(A).
