PREVIOUS MINOR VIOLATIONS NOT CORRECTED FROM REPORT #159533 DATED 8/12/09. 35-MUST READJUST THE DOOR SWEEP IN FRONT ENTRANCE DOOR. #40- MUST PROVIDE THERMOMETER INSIDE THE WALK IN FREEZER AND WALK IN COOLER AND MUST BE VISIBLE. CITATION ISSUED SERIOUS 7-42-090

VIOLATION #29 HAS BEEN CORRECTED.

Adequate and convenient hand washing facilities shall be provided for all employees. FOUND EXPOSED HAND SINK IN THE REAR PREP AREA,NOT ACCESSIBLE AND CONVENIENT FOR THE EMPLOYEES. EXPOSED HAND SINK IS BEING USED AS STORAGE FOR PERSONAL THINGS SUCH AS HAND BAG, APRON,HAT ,AND PIPE. INSTRUCTED MANAGER THAT EXPOSED HAND SINK IS USED FOR HAND WASHING ONLY ,NOT FOR STORAGE. CITATION ISSUED CRITICAL. MANAGER REMOVED ALL THE ITEMS FROM THE EXPOSED HAND SINK.
All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. MUST RE CAULK THE SPLASH GUARD OF THE WASHBOWL SINK IN THE MENS WASHROOM. CAULKING WORN OUT. MUST REMOVE THE CARDBOARD FROM THE STORAGE SHELVES IN THE REAR AREA.
The walls and ceilings shall be in good repair and easily cleaned. CEILING VENT COVER INSIDE THE MENS AND LADIES WASHROOM WITH MINIMAL DUST BUILT UP. INSTRUCTED TO CLEAN AND MAINTAIN.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. MUST CLEAN IN DETAIL THE FLOOR UNDER ALL THE WIRE SHELVINGS IN THE REAR AREA.
All parts of the food establishment and all parts of the property used in connection with the operation of the establishment shall be kept neat and clean and should not produce any offensive odors. CLUTTER IN THE REAR BY THE EXIT DOOR. MUST REMOVE ALL THE CARDBOARD AND EMPTY BOXES IN THE REAR AREA BY THE EXIT DOOR .

Floors under heavy equipment, cooking equipment, and under storage shelving not clean need detailed cleaning(corners), floors by hot water heater need detailed cleaning.
Walls in rear kitchen area by 3 compartment sink need cleaning.
Light shields not clean need cleaning.
Exhaust vents(ventilation)in walk in cooler not clean need cleaning, exhaust vent in womans bathroom in state of disrepair, shall be repair/replaced.

Non food contact surfaces of prep table lower shelving not clean, need detailed cleaning(crevices)
Floors in rear storage areas by mop sink, and bathrooms not clean, need cleaning(detailed).
Stock items must be stored six inches off of floor to prevent pest harborage.
Exhaust vents(ventilation) in kitchen prep areas, bathrooms not clean, need detailed cleaning.
'MENUMASTER' microwave oven upper inside shelving/guard in state of disrepair, shall be replaced/repaired.
