Violation descriptions and comments

The walls and ceilings shall be in good repair and easily cleaned. Bathroom vent, fan guards of most coolers need cleaning due to dust build-up.
The flow of air discharged from kitchen fans shall always be through a duct to a point above the roofline. Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. Must replace condensation hose of reach-in freezer that's in poor repair.
All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. Must provide a plashguard on both sides of handsink (located between 2-prep tables). Must fix rubber gaskets of cooler drawers at serving line & repair an interior panel of rear cooler door that's popping out. Must provide adequate amount of sink stoppers at 3-comp sink.
Toilet rooms shall be completely enclosed and shall be vented to the outside air or mechanically ventilated. Bathroom door needs a self-closing device.

FOUND PREP REACH IN COOLER(UNDER GRILL)NOT MAINTAINED AMBIENT AIR TEMPERATURE AT 55.0 F WITH CHICKEN, BEEF, ETC. INSIDE COOLER. INSTRUCTED MANAGER TO REPAIR COOLER, AMBIENT AIR TEMPERATURE MUST BE 40 F OR BELOW. CITATION ISSUED.
All cold food shall be stored at a temperature of 40F or less. FOUND THE FOLLOWING POTENTIALLY HAZARDOUS FOODS HELD AT IMPROPER TEMPERATUES BETWEEN 47.0 F AND 52.5 F- CORNED BEEF, BEEF BURGERS, TURKEY, TURKEY BURGERS, BOILED EGGS, CHEESE, CHICKEN. INSTRUCED MANAGER ALL POTENTIALLY HAZARDOUS FOODS MUST BE HELD AT 40 F OR BELOW IF HELD COLD. CITATION ISSUED. FOODS VOLUNTARILY DISCARDED AT THIS TIME, VALUED AT $15.00, WEIGHT 13 LBS.
