Containers and utensils shall be inverted, covered, or otherwise protected from contamination until used. All single-service drinking straws and containers shall be discarded immediately after use.ALL MULTI-USE UTENSILS,DISHWARE NOT PROPERLY STORED,INVERTED TO PREVENT CONTAMINATION BEFORE USE.
All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair.FOUND GASKET AT KITCHEN WEST 2 DOOR COOLER IN POOR REPAIR,RUST ON SHELVING INSIDE THE WALK-IN-COOLER.
Shielding to protect against broken glass falling into food shall be provided for all artificial lighting sources in preparation, service, and display facilities.REPLACE BROKEN LIGHT SHIELD IN KITCHEN.

Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. MUST REPAIR SLOW DRAINING EXPOSED HAND BOWL IN KITCHEN AREA, AND LEAKING PIPES UNDER GARBAGE DISPOSAL
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. MUST CLEAN FLOORS UNDER AND BEHIND ALL EQUIPMENT THROUGHOUT PREMISES
All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily. All utensils shall be thoroughly cleaned and sanitized after each usage. MUST CLEAN VENTS ABOVE COOKING EQUIPMENT

Cutting boards with deep, dark grooves must be sanded/bleached or replaced and remove peeling paint from the rear dry storage shelvings.
The interior of light shields throughout the kitchena areas require a detail cleaning and replace burnt-out light bulbs were noted.
The following requires a detail cleaning exterior surfaces of the freezer units,shelvings inside the walk-in-cooler,upper panel of ice machine,pipes beneath the garbage disposal unit remove grease build-up etc.
All food stored in coolers must be dated & labeled,Any open cans of food must be transferred into regular containers.

We observed sewage waste water backing up from floor drain onto prep kitchen floor at the 3 comp sink area and waste water backing up from disposal unit located next to the dishmachine also exposed handsink is clogged very slow drain same area.Must provide all the necessary plumbing repairs per city code.Critical violation 7-38-030.No citation issued.
FOOD (ICE) NOT PROTECTED DURING STORAGE INSIDE OF THE ICE MACHINE. OBSERVED BLACK SLIMY SUBSTANCE ON INTERIOR SURFACES OF THE ICE MACHINE ON THE PREMISES. MANAGEMENT INSTRUCTED TO TURN OFF MACHINE, REMOVE ICE AND HAVE INTERIOR OF ICE MACHINE CLEANED AND SANITIZED.THE ICE MACHINE NOW TAGGED HELD FOR INSPECTION .MUST FAX LETTER TO THE CDPH 312-746-4240 FOR TAG REMOVAL ATT:A.LOPEZ. SERIOUS VIOLATION 7-38-005A. NO CITATION ISSUED.
Three (3) doors to the premises must be insect/rodent proofed all accross the bottoms and pest control log book must be complete with a map locator identifying all pest control services to the premises.Serious violation 7-38-020.No citation issued.
We observed that the employee toilet room not maintained said toilet unable flush at this time.Serious violation 7-38-030.
Floors beneath the 3 comp sink area bar locatin is dirty,clean floors in detail also regrout floor tiles same area.
The interiors of the bar cooler require a detail cleaning.
3 sink stoppers are needed at the 3 compartment sink bar location and remove aluminum foils from the kitchen cooking equipments all surfaces must be smooth,easily cleanable.
A exposed handsink with soap and handtowels is needed at bar location for the use of handwashing.Critical violation 7-38-030.No citation issued.
Detail clean dusts accumulations on the ventilation covers throughout the premises and the ventilation in the toilet rooms is not working also the hood/filters over the cooking equipments require a detail cleaning.
Must provide and ensure adeaquate hot running water under city pressure in sinks throughout the premises.
