Containers and utensils shall be inverted, covered, or otherwise protected from contamination until used. All single-service drinking straws and containers shall be discarded immediately after use. Found food containers not stored properly on shelving units.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. Found floors not clean at main kitchen.

Equipment and utensils should get proper exposure to the sanitizing solution during the rinse cycle. Bactericidal treatment shall consist of exposure of all dish and utensil surfaces to a rinse of clean water at a temperature of not less than 180F. LOW TEMPERATURE DISHMACHINE NOT MAINTAINED, 0 PPM CHLORINE WHILE ON. INSTRUCTED MANAGER DISHMACHINE MUST SANITIZE AT 100 PPM CHLORINE. ALL MULTI-USE UTENSILS MUST TEMPORARILY BE WASHED, RINSED AND SANITIZED IN 3-COMPARTMENT SINK. CITATION ISSUED. TICKET #H000063473 16.

NO COOLERS OR HOT HOLDING UNITS FOUND INSIDE MOBILE FOOD DISPENSER AT THIS TIME. INSTRUCTED MANAGER ALL COOLERS MUST REACH AMBIENT AIR TEPERATURE OF 40 F OR BELOW AND HOT HOLDING UNIT MUST BE ABLE TO REACH 180 F.
All food establishments shall be provided with an adequate supply of hot and cold water under pressure properly connected to the city water supply. NO RUNNING HOT OR COLD WATER INSIDE MOBILE FOOD DISPENSER AT THIS TIME. INSTRUCTED MANAGER TO PROVIDE RUNNING HOT AND COLD WATER INSIDE VEHICLE.
In food establishments, there shall be adequate sewage and waste water disposal facilities that comply with all the requirements of the plumbing section of the Municipal Code of Chicago. NO WASTE WATER RETENTION TANK INSIDE/ON MOBILE FOOD DISPENSER AT THIS TIME. MUST PROVIDE AND INSTALL WASTE WATER TANK.
Adequate and convenient hand washing facilities shall be provided for all employees. NO HANDWASH BOWL INSIDE MOBILE FOOD DISPENSER AT THIS TIME. MUST PROVIDE HANDWASH BOWL IN VEHICLE.
All necessary control measures shall be used to effectively minimize or eliminate the presence of rodents, roaches, and other vermin and insects on the premises of all food establishments, in food-transporting vehicles, and in vending machines. OBSERVED OVER 20 LIVE SMALL FLIES INSIDE MOBILE FOOD DISPENSER AT THIS TIME. MUST DETAIL CLEAN AND SANITIZE INTERIOR OF VEHICLE/ELIMINATE PEST ACTIVITY.
