The establishment does not have an employee health policy or can not provide proof of policy. No signed employee interview and reporting forms exist regarding the prevention of the transmission of diseases through food by infected food employees. - Adequate handwashing sinks properly supplied and accessible
No paper towels at handwashing sink located next to the 3-compartment sink. (Corrected On Site) - Insects, rodents, & animals not present
Rodent droppings has been observed at serveral places through out the facility, particularily in the upstair storage rooms. - Contamination prevented during food preparation, storage, & display
Dry flour stored in opened paper bag is exposed to contaminaton. - Physical facilities: installed, maintained, & clean
Ceiling paint peeling at the upstair storage room. The cove bases are not in good repair in the upstair storage rooms.
