The facility menu lacked the proper consumer advisory for their brunch, lunch, and dinner menu items that are served raw or undercooked.
An employee near the expo area was observed wiping their gloved hands on a common towel and then handling clean utensils.
Green chili was 130�°F and black beans were 127�°F in the cook line hot-holding unit.
A shut off (solenoid) was present downstream of the dish machine atmospheric vacuum breaker under the drain board.
No soap or paper towels were provided at the bar hand sink.
Two fly strips were present above the expo area preparation table.
A chemical spray bottle under the womens restroom hand sink was not labeled. A chemical spray bottle under the womens restroom hand sink was mislabeled as bleach when it was glass cleaner.
Food on the cook line cold-top refrigerator was not covered.

Tape was used to repair a scale on the preparation table island shelf. The cook line reach-in refrigerator condenser was leaking. The door gasket on the cold-top reach-in refrigerator across from the hot-holding unit was torn. The door of the reach-in freezer at the bar was broken and did not close.
The blade on the can opener on the preparation table was soiled with food debris. The interior basin of the stove oven was soiled with food debris. The interior plate of the ice bin in the back of the kitchen was moldy. The interior plate of the service station ice bin was moldy.
The exterior of the dish machine was soiled with hard water. The shelves in the dry storage area were soiled with food debris. The can opener on the preparation table was soiled with food. Several door gaskets on the cook line cold-top reach-in refrigerators were soiled with food debris.
An in-use utensil in the expo area was stored in 66�°F in standing water. The scoop in the sugar container in the dry storage area did not have a handle.
Coffee filters in the service station were not covered.
An ASSE 1022 dual check valve was not provided on the water line supplying the tea maker at the wait station. The caulking on the service station hand sink was not adequately sealed to the wall.
The wall behind the dish machine and behind the cook line hot-holding units was soiled with food debris. The wall in the dry storage area was falling apart.
Several lights above the kitchen preparation table were not functioning properly.
