Observed two other CFM certificates and manager on shift, Guy, stated they work at other locations in Live Oak and Lake City. This violation must be corrected by October 14, 2014. Observed manager sweep and remove vermin. This violation must be corrected by Monday, April 28, 2014. **Warning** Open callback 4/28/14: Observed 2 dead roaches under shelving in dry storage and 1 dead roach under prep table near fryers in kitchen. Approval to add violation to existing admin complaint approved per Supervisor Belinda Talley. Open callback 613/14: Observed two dead roaches under shelving in dry storage room with door leading to exterior. Observed 2 dead roaches at corner of baseboard beneath slicer table in kitchen. Observed two dead roaches beneath dish machine in ware washing area. Manager stated that the pest control company had visited this morning (viewed Terminex receipt with date of 6/13 at 9:55) and that there is a problem with an exterior city sewer drain that they believe is leading to the increase in vermin activity. Manager stated they are trying to work with the city of Madison to remedy the problem.

Observed Friday special advertised as "all you can eat catfish" on sign at road and on dry erase board at hostess stand. Observed catfish filet and catfish dinners advertised on restaurant menu. Observed inspector escorted through routine inspection by Guy McCaskill, manager, and when asked by inspector to view the catfish establishment uses, inspector was led to walk-in freezer and shown 6 cases of frozen fish labeled as "pangasius swai" and inspector was advised that was the product used for catfish. Observed no other fish products stored in freezer. Advised establishment that pangasius swai may not be labeled and sold as "catfish" that only North American Freshwater Catfish may be sold and labeled as "catfish". Advised establishment that if they wish to continue to sell the pangasius swai, they may label it as Swai, Panga or Pangasius on their menus and boards or to purchase North American Freshwater Catfish to sell as advertised. **Admin Complaint**
