- Observed C02 tank not secured. In ice machine room.
- Observed gasket missing at two door reachin cooler next to walkin freezer under microwave (right side).
- Critical - Observed handwash sign at bar missing also soap and paper towels at 2nd sink at bar.
- Critical - Observed hot and cold water shut off at 3 compartment sink at bar. Corrective action turned on.
- Critical - Observed interior of ice machine soiled (near ice chute) corrective action cleaned (no ice in machine yet). No salmon on menu or other fish requiring parasite destruction records.
- Observed missing waste line at soda gun at bar (both).
- Critical - Observed no proof of required employee training. New employees must be trained within 60 days of hire.
- Observed no trash cans in establishment.
- Critical - Observed no vacuum breaker on splitter with hose attached at mop sink.
- Observed old labels stuck to clean containers. On shelf with clean dishes outside walkin cooler.
