It's a strange phenomenon that people tend to view potential guilt, in part, through the lens of considerations such as wealth, physical attractiveness, gender, etc., when in the end people are just people.
I think that in this case, they're shocked that a person who went all the way through med school and completed his residency was this unethical and this disturbed. One would have thought that someone like this would have been weeded out in the process. The fact that he wasn't is more than concerning.

I did not see his mug shots, but his girlfriend looks like she belongs in a horror movie.
I didn't say that she physically resembled Wanda Barzee, but their actions are the same. Wanda helped her boyfriend kidnap Elizabeth Smart, and encouraged her boyfriend to rape her. Apparently this woman did the same.

Wasn't one of the 1st "Craigslist Killers", Philip Markoff, a medical student at Boston University at the time of his arrest?

How could they weed him out. Medical schools probably do interviews but that doesn't reveal anything. Psychopaths are very good at fooling people. They are often popular and have lots of friends.
Amazing that his girlfriend was complicit in this. What’s the matter with her? She’s like that old bat Barzee who helped kidnap Elizabeth Smart.
It is not unusual that men who kidnap women will have a female accomplice. It helps them because women will trust other women, thinking a woman won't harm them.
And since Philip Markoff was mentioned, I've often wondered if his GF had at least some knowledge of the crimes he was committing. Or, at the very least, had suspicions. He kept a lot of his crime stuff in the bedroom of the home they shared. She never noticed any of it? Since he committed suicide in prison, we'll never know.

Wasn't she arrested on the same charges?
Possible Dr supplying / enabling GF drug habit as well.

I don't think Phillip Markoff's fiance was arrested, although I'm sure she was questioned by the police. She was a medical student also and I'm sure she would have had the same access to drugs he did. But drugs weren't a factor in the murders he committed. He killed women to rob them of their money, to pay off his debts. He gambled a lot, had student loans, etc.
