I've never heard of it being a sign of heartworms. Is she on heartworm preventative? Has she been to the vet since you've had her? If not I'd take her for a checkup. It's possible that they were not truthful with you about her age.

Some dogs just grey early. My female GSD was grey at 2.

It's not "gray". It's what we call "frost". Some of them are that way from puppyhood.

Default grey face on Boston Terrier

Jack's muzzle has frosted in the two years that we have had him and he is 4.
