Is he crate trained? Or can he be gated in a smaller area with no rugs, beds, etc? It sounds like you are leaving him loose while you are out and with all the urine smell probably still in the hosue (especially the rug), probably along with some separation anxiety it is too much for him. Also, if he is getting on the bed, definitely close the bedroom door. When one of my older boys started to have accidents, it sounds a bit nuts but I got one of those belly bands at Petsmart then put a couple of depends inside - two of them would soak up just about anything. Gross I know, but he couldn't be crated because he got used to being out of it (my fault) but it kept the urine from really getting all over everything. Also, if you are using an enzyme cleaner to clean up, make sure you let it soak for at least 30 minutes. It won't work unless it sits for awhile.

Is this your first dog? Your first puppy?

Is he generally "house trained" when you ARE home?
Do you observe any behaviors when he needs to go out?
Do you take him out for exercise regularly? 2-3 times per day?

Your first step ought to be isolating him in a smaller area when you aren't home - either a crate or a smaller room that has no rugs or carpeting. One of three things is happening: he's bored, he's scared, or he can't hold it that long. Finding out which is triggering it will help you come up with the best solution.
