Could be physical and /or psychological, but from what you've described I'm thinking you both need to get into it mentally to understand the physical, psychological, and emotional connection you are looking to attain by the intimacy. As a few others have mentioned, experiment more with being able to relieve and release yourself. See what 'mood' environment setting works - then convey that to your partner.
For me, the lube allergy caused lots of little sores that felt like I was being ripped open, and I would bleed from it. It was a fairly basic lube we were using, I think it was walmart's generic. I already knew I was allergic to Astroglide, but we'd used the walmart lube before without a problem. Depo Provera can cause a loss of desire and vaginal dryness and irritation, so that might be a consideration.
I would suggest having a drink or two next time you're going to attempt sex. Get the lube thing figured out first though.
